宫颈癌根治术后33例患者远期下尿路功能特征分析

摘　　要：目的分析宫颈癌患者术后远期下尿路功能特征,探讨该类患者术后远期尿动力学检查的临床意义。方法选取我院收治的33例早期宫颈癌患者,应用尿动力学方法进行术前后下尿路功能检测,分析宫颈癌患者术后远期下尿路功能的变化特点。结果宫颈癌患者术后远期的膀胱初感容量以及残余尿量均明显高于术前(P<0.05),而膀胱顺应性、最大尿流率、最大尿流率时逼尿肌压力均明显低于术前(P<0.05)。同时,术后1年低顺应性膀胱、膀胱逼尿肌损伤、感觉缺失及尿失禁的发生率均明显高于术前(均P<0.05)。结论宫颈癌患者术后可较长时间存在下尿路功能改变,并可表现为低顺应性膀胱、逼尿肌损伤、感觉缺失与尿失禁。尿动力学检测能够客观评价该类患者术后远期的下尿路功能,并指导其临床诊治。Objective To analyse the long-term functional characteristics of lower urinary tract in patients with cervical cancer after radical hysterectomy,and explore the clinical significance of postoperative long-term urodynamic study in these patients.Methods Thirty-three patients with early cervical cancer in our hospital were detected the function of lower urinary tract by urodynamic study pre and post-operation,in order to analyse the characteristics of long-term functional change of lower urinary tract after operation.Results The first sensation and post voiding residual of bladder increased significantly（P〈0.05）,whereas the compliance,the maximum flow rate and the pressure at the maximum flow rate decreased obviously after radical hysterectomy（P〈0.05）.At the same time,the incidence of low compliance bladder,detrusor injury,sensory deficiency and urinary incontinence was significantly higher after one year following surgery（P〈0.05）.Conclusion Patients with cervical cancer may experience changes in the function of the lower urinary tract for a long time after surgery,and may present with low compliance bladder,detrusor muscle injury,sensory deficiency and urinary incontinence.Urodynamic study can objectively evaluate the long-term postoperative function of the lower urinary tract in these patients and guide their clinical diagnosis and treatment.
