摘　　要：目的:探讨非酒精性脂肪肝对吉西他滨联合顺铂治疗肺癌疗效的影响。方法:将肺癌患者按是否患非酒精性脂肪肝分为观察组和对照组,两组患者均采用吉西他滨联合顺铂药物治疗,分析两组患者疗效及不良反应发生情况差异。结果:(1)观察组完全缓解率为6.3%,部分缓解率为28.1%,总有效率为34.4%,对照组完全缓解率为32.1%,部分缓解率为42.9%,总有效率为75.0%,两组资料有效率差异有统计学意义(P<0.05);(2)两组患者均出现骨髓抑制、消化系统反应、过敏反应、神经毒性、心脏毒性、肝脏毒性等不良反应,且两组资料除肝脏毒性外,不良反应发生率差异无统计学意义(P>0.05),试验组肝脏毒性发生率高于对照组(P<0.05)。结论:非酒精性脂肪肝对吉西他滨联合顺铂治疗肺癌疗效有影响,化疗前应采取措施治疗或控制病情。Objective： To investigate the effect of non-alcoholic fatty liver （NAFLD） on the efficacy of gemcitabine combined with cisplatin in the treatment of lung cancer.Methods： The patients with lung cancer were divided into observation group and control group according to whether they had non-alcoholic fatty liver.Two groups were treated with gemcitabine combined with cisplatin, while analyzing the difference in curative effect and adverse reactions.Results： Firstly, in the observation group, the CR rate was 6.3%, the PR rate was 28.1%, and the total effective rate was 34.4%.However, in the control group, the CR rate was 32.1%, the PR rate was 42.9%, and the total effective rate was 75.0%, with a significant difference between the two groups（P〈0.05）.Secondly, adverse reactions including bone marrow suppression, digestive system reaction, allergic reaction, neurotoxicity, cardiac toxicity and liver toxicity were observed, there was no statistically significant difference in the incidence of adverse reactions between the two groups（P〉0.05）, except for hepatictoxicity, which occurred in the observation group was higher than that the control group（P〈0.05）.Conclusion： NAFLD has an effect on the efficacy of gemcitabine combined with cisplatin in the treatment of lung cancer.Effective measures should be taken to treat or control the disease before chemotherapy.
