外伤性晶状体全脱位56例手术治疗及远期随访中治疗的效果

摘　　要：目的探讨外伤性晶状体全脱位手术治疗效果及随访中治疗的远期疗效。方法回顾性分析56例56眼外伤性晶状体全脱位患者的临床资料,56眼均采用玻璃体切除联合手术治疗,随访中对并发症及需要进一步处理的病情及时进行个性化手术治疗,并经过15~34个月远期随访观察。结果所有患者均成功切除全脱位的晶状体,随访止点矫正视力≥0.3者(26眼)46.43%,矫正视力≥0.05者45眼(80.36%)。术后矫正视力与术前比较差异均有统计学意义(P<0.05)。随访止点眼压在正常范围55眼(98.21%),平均眼压(16.24±4.73)mm Hg(1 mm Hg=0.133 k Pa),无人工晶体脱位、移位发生;但眼球萎缩1眼,角膜失代偿1眼。结论眼外伤晶状体全脱位治疗早期采取玻璃体切除联合手术治疗,对减少并发症、保持视功能至关重要。手术的个性化设计及技巧是视力恢复、减少并发症的保障。术后随访中并发症及需要进一步处理的病情的及时治疗对有效视功能的恢复起关键作用。Objective To investigate the long-term effect of surgical treatment plus treatment in follow-up for traumatic complete lens dislocation. Methods The clinical data of 56 cases（56 eyes） with traumatic complete dislocation of the lens were retrospectively analyzed,which 56 eyes were treated with early vitrectomy combined with surgical treatment,and the complications were timely treated. The follow-up time ranged from 15 to 34 M.Results All the patients were successfully removed the lens. In follow up stop point,vision was with 26 eyes better than 0.3（46.43%）,and 45 eyes better than 0.05（80.36%）. The postoperative corrected visual acuity was significantly different from that before operation. The intraocular pressure in the follow-up point was 55 eyes（98.21%） within the normal range,and the mean intraocular pressure was（16.24±4.73）mm Hg,no IOL lwxation or translocation were found;but there was 1 eye with atrophy and 1 eye with corneal decompensation. Conclusion Early vitrectomy combined with surgery is very important for reducing complications and maintaining visual function in the patients with complete luxation of ocular lens. Personalized surgical design and skills are the guarantee of vision recovery and reducing complications. The follow-up treatment of complications plays a key role in the recovery of effective visual function.
