加味八珍汤联合STARR术治疗气阴两虚型直肠前突性便秘疗效观察

摘　　要：目的观察加味八珍汤联合STARR术治疗气阴两虚型直肠前突性便秘的临床疗效。方法将60例直肠前突气阴两虚证患者随机分为观察组和对照组,每组30例。观察组予以加味八珍汤联合STARR术治疗,对照组予以单纯STARR术治疗。观察2组综合疗效、临床症状改善情况、术后24 h疼痛情况、术后6个月复发率。结果最终完成临床观察52例,观察组27例,对照组25例; 2组患者治疗后便秘、排便不畅感和肛门下坠感积分及总积分均明显降低(P均<0. 05),观察组上述评分均明显低于对照组(P均<0. 05);观察组术后24 h疼痛VAS评分分级情况明显低于对照组(P <0. 05),总有效率明显高于对照组(P <0. 05),术后6个月的复发率显著低于对照组(P <0. 05)。结论加味八珍汤联合STARR术可明显改善气阴两虚型直肠前突性便秘患者临床症状,并减轻术后疼痛,降低远期复发率。Objective It is to observe the clinical efficacy of modified Bazhen Decoction combined with STARR surgery in the treatment of rectal prolapse constipation of syndrome of Qi and Yin deficiency. Methods Sixty patients with rectal prolapse constipation of syndrome of Qi and Yin deficiency were randomly divided into observation group and control group, 30 cases in each group. The observation group was treated with modified Bazhen Decoction combined with STARR surgery, and the control group was treated with STARR alone. The comprehensive efficacy, improvement of clinical symptoms, pain at 24h after operation, and recurrence rate at 6 months after operation were observed in the two groups. Results 52 cases were finally observed in this clinical observation, 27 cases in the treatment group and 25 cases in the control group. The scores of constipation, defecation sensation and anal fall sensation and total scores were significantly decreased after treatment in the two groups（ P 〈0.05）. The scores above in the observation group were significantly lower than those in the control group （ P 〈0.05）. The pain VAS scores at 24h after surgery in the observation group were lower than that in the control group （ P 〈0.05）, and the total effective rate was higher than that in the control group （ P 〈0.05）. The recurrence rate at 6 months after operation in the observation group was significantly lower than that of the control group （ P 〈0.05）. Conclusion Modified Bazhen Decoction combined with STARR surgery can significantly improve the clinical symptoms of patients with rectal prolapse and constipation of syndrome of Qi and Yin deficiency, and reduce postoperative pain and reduce the long-term recurrence rate.
