微切口白内障超声乳化联合双焦点散光人工晶体植入术有效性临床观察

摘　　要：目的比较双焦点散光人工晶体与单焦点人工晶体植入术后的短期疗效。方法选取2014—2016年该院收治的行白内障联合人工晶体植入术患者25例(30眼),分为A组[12例(15眼)]和B组[13例(15眼)]。A组植入双焦点散光人工晶体,B组植入单焦点人工晶体。观察2组患者术后1、6个月未矫正远视力(UDVA)、矫正远视力(CDVA)、裸眼近视力(UNVA)、人工晶体偏移、角膜散光、角膜曲率、对比敏感度(CS)及脱镜率等。结果 2组患者UDVA、CDVA、低中频区(3、6、12 cpd)术后CS比较,差异均无统计学意义(P>0.05);2组患者UNVA、术后角膜散光、高频区(18 cpd)术后CS比较,差异均有统计学意义(P<0.05);2组患者术后脱镜率均为100.0%。结论双焦点散光人工晶体对白内障合并散光患者的手术效果较好,不仅提供了较好的近视力,使患者摆脱了老花镜的烦恼,基本矫正术前散光对患者生活及视觉质量的影响,为患者提供了高水平的视觉质量,且术后人工晶体具有较好稳定性,故白内障手术联合植入双焦点散光人工晶体的手术效果具有一定的短期预见性。Objective To compare the short-term efficacy of bifocal astigmatism-corrected intraocular lens and mono-focal intraocular lens（IOL）implantation. Methods Twenty-five patients（30 eyes）with cataract phacoemulsification com-bined with bifocal astigmatism intraocular lens implantation in the hospital during 2014—2016 were selected and divided intothe group A（12 cases,15 eyes）and group B（13 cases,15 eyes）.The group A was treated with bifocal astigmatic artificial lensimplantation,while the group B was treated by monofocal IOL implantation. The uncorrected distance vision（UDVA）,correcteddistance vision（CDVA）,uncorrected near vision（UNVA）,intraocular lens deviation,corneal astigmatism,corneal curvature,contrast sensitivity（CS）and de-glass rate at postoperative 1,6 months were observed in the two groups. Results The UDVA,CDVA,and postoperative CS at low-medium frequency region（3,6,12 cpd）had no statistical differences between the twogroups（P〉0.05）.The UNVA,postoperative corneal astigmatism,and postoperative CS at high frequency region（18 cpd）had statistical differences between the two groups（P〈0.05）. The postoperative de-glass rate was 100.0% in the two groups.Conclusion The bifocal astigmatism correcting intraocular lens has a good surgical effect on cataract patients with astigma-tism,which not only provides good near vision,but also makes the patient out of reading glasses troubles,and basically correctsthe influence of preoperative astigmatism on the patient′s life and vision quality,provides a high level version quality for the pa-tients,moreover postoperative IOL has better stability,therefore,the surgical effect of cataract surgery combined with implanta-tion of bifocal astigmatism crystals has a certain short-term predictability.
