CLAG方案和MEA方案治疗复发/难治急性髓系白血病的疗效比较

摘　　要：目的探究分别采取MEA的方案和CLAG方案对于治疗复发/难治的急性髓系白血病(RR-AML)所产生的疗效并进行对比。方法选取2014年1月—2018年1月期间在该院接受治疗的RR-AML患者89例作为研究对象进行研究,根据患者的治疗方案的不同分为对照组40例和观察组49例,其中对照组患者采取MEA的方案,观察组患者采取CLAG方案,对比两组患者的治疗安全性以及临床疗效。结果两组患者的CR、PR、RR以及NR相比差异无统计学意义(χ~2=1.185,0.744,1.277,1.136,P>0.05);观察组患者的MOS为285 d,MPSF为231 d,均显著高于对照组患者(t=6.342,7.391,P<0.05);复查时发现观察组患者发生发热28例(70.00%),肺部感染12例(30.00%),粒细胞缺乏35例(87.50%),与对照组患者相比差异无统计学意义(χ~2=1.384,1.542,1.779,P>0.05)。结论采取CLAG治疗方案对于RR-AML患者具有的救助效果以及安全性同MEA方案效果相近,其带来的不良反应具有可控性,可期望获得造血干细胞移植机会来挽救生命,值得临床参考。Objective To compare the curative effect of CLAG plan and MEA plan in treatment of recurrence/refractory acute myeloid leukemia.Methods 89 cases of patients with RR-AML in our hospital from January 2014 to January2018 were selected for study,and divided into two groups according to different treatment plans,the control group with40 cases used the MEA plan,while the observation group with 49 cases used the CLAG plan,and the safety and clinical curative effect were compared between the two groups.Results The differences in the CR,PR,RR and NR between the two groups were not obvious（χ2=1.185,0.744,1.277,1.136,P〉0.05）;and the MOS 285 d and MPSF 231 d in the observation group were obviously higher than those in the control group（t=6.342,7.391,P〈0.05）,at reexamination,and in the observation group,there were 28 cases with fever（70.00%）,12 cases with lung infection（30.00%）,35 cases with agranulocytosis（87.50%） the differences in the incidence rate of adverse reactions between the observation group and the control group were not statisically significant（χ2=1.384,1.542,1.779,P 〉0.05）.Conclusion The treatment effect and safety of CLAG treatment plan are similar to those of MEA plan for RR-AML patients,and the adverse reactions can be controlled,and the transplantation chance of hemopoietic stem cells can be expected to save lives,and it is worth reference.
