摘  要:目的通过计算机软件的图像重建技术(3Dslicer,www.slicer.org),将术前CT,MRI断层扫描图像转换为3D虚拟可视图像,可用于模拟手术入路并了解蝶窦内解剖情况。方法选取2017年1月—2017年6月在邢台市人民医院神经外三科接受手术治疗的垂体瘤患者6例,常规术前检查行垂体MRI平扫强化,蝶窦CT,获取图像文件后导入计算机,使用3Dslicr软件重建后,可得到蝶鞍区3D虚拟图像。结果 6例手术顺利,术中图像与术前3D重建图像基本一致,可以指导手术,避免手术方向的偏离,降低了大血管损伤的几率。结论通过术前图像重建以及3D虚拟现实技术的应用,可以在术前帮助医生更好的理解MRI,CT断层图像,熟悉蝶窦内解剖情况,避免手术路径偏航,降低了手术中颈动脉损伤的风险。Objective To transform the CT and MRI Tomography image before surgery into 3 D virtual visual images that can be used to simulate surgical approach and know the anatomy of the sphenoid sinus.Methods 6 cases of patients with hypophysoma for surgical treatment in our hospital from January to June 2017 were selected,and the patients were for routine preoperative examination and MRI plain scan of pituitary and sphenoid sinus C,and the images and documents were obtained and induced into the computer,and the 3 D virtual images in the sella region could be obtained after the reconstruction of 3 Dslicr software.Results 6 cases had successfully finished the surgery,and the intraoperative images were basically consistent with the preoperative 3 D reconstruction images,which could guide the surgery,avoid the deviation of surgical direction and reduce the probability of large vascular injuries.Conclusion The application of preoperative image reconstruction and 3 D virtual reality technology can help doctors better understand the application of MRI,CT tomographic images,master the anatomy of the sphenoid sinus,avoid the deviation of surgical direction and reduce the risk of intraoperative carotid artery injuries.
