降藓方治疗糜烂型口腔扁平苔藓阴虚内热证临床观察

摘　　要：目的观察降藓方治疗糜烂型口腔扁平苔藓(OLP)阴虚内热证的临床疗效。方法将66例糜烂型OLP阴虚内热证患者按照随机数字表法分为2组。对照组35例予西医常规治疗;治疗组31例予降藓方治疗。2组均14 d为1个疗程,连续治疗2个疗程,治疗后6个月随访。比较2组近期、远期疗效;比较2组治疗前后疼痛视觉模拟评分法(VAS)评分、充血面积及糜烂面积变化,2组治疗前及随访时中文版口腔健康影响程度量表(OHIP)评分变化。结果治疗组近期总有效率86.67%,对照组近期总有效率78.79%,治疗组近期疗效优于对照组(P<0.05)。治疗组远期总有效率72.41%,对照组远期总有效率65.63%,治疗组远期疗效优于对照组(P<0.05)。2组治疗后疼痛VSA评分、充血面积及糜烂面积均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组随访时OHIP评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。结论降藓方治疗糜烂型OLP阴虚内热证疗效确切,能减轻患者疼痛,改善症状,提高生活质量。Objective To observe the effects of Jiangxian decoction on erosive oral lichen planus （OLP） with Yin deficiency and internal heat syndrome. Methods 66 patients with erosive OLP with Yin deficiency and internal heat syndrome were divided into two groups according to the random number table method. 35 patients in control group were treated by conventional western medicine. 31 patients in treatment group were treated by Jiangxian decoction. Both groups were treated for 14 days for 1 course, 2 courses for continuous treatment, and followed up after 6 months treatment. The short-term and long-term effects of the two groups were compared. The pain visual analogue scale （VAS） score, the hyperemic area and the erosion area were compared before and after treatment. The changes in the Chinese version of the Oral Health Impact Scale （OHIP） before treatment and during the follow-up were compared between the two groups. Results The short-term total effective rate was 86.67 % in the treatment group and 78.79 % in the control group. The short-term effect of the treatment group was better than that of the control group （ P 〈0.05）. The long-term total effective rate was 72.41 % in the treatment group and 65.63%in the control group. The long-term effect of the treatment group was better than that of the control group （ P 〈0.05）. The VSA score, hyperemia area and erosion area of the two groups were lower than those before treatment （P〈 0.05 ）, and the treatment group decreased more significantly （ P 〈0.05）. The OHIP scores of the two groups were lower than those before the treatment （ P 〈0.05）, and the treatment group decreased more significantly （ P 〈0.05）. Conclusion Jiangxiao decoction is effective in treating erosive OLP with Yin deficiency and internal heat syndrome, which can alleviate pain, improve symptoms and improve quality of life.
