摘  要:目的探讨改良微骨孔完全显微血管减压术与传统显微血管减压术治疗原发性三叉神经痛(ITN)临床疗效。方法选取2014-08—2016-01安阳市人民医院82例ITN患者,通过随机数字表法分为对照组和研究组各41例。对照组采用传统显微血管减压术,研究组采用改良微骨孔完全显微血管减压术。统计对比2组手术情况、临床疗效、并发症发生率,分析手术前后2组三叉神经周边部分脑池容积变化情况。结果研究组手术用时、住院时间少于对照组,差异有统计学意义(P<0.05);研究组治疗有效率95.12%(39/41)高于对照组78.05%(32/41),差异有统计学意义(P<0.05);术前2组三叉神经周边部分脑池容积比较,差异无统计学意义(P>0.05),术后2组三叉神经周边部分脑池容积均较术前增大,且研究组三叉神经周边部分脑池容积大于对照组,差异有统计学意义(P<0.05);研究组并发症发生率12.20%(5/41)低于对照组31.72%(13/41),差异有统计学意义(P<0.05)。结论改良微骨孔完全显微血管减压术治疗原发性三叉神经痛效果优于传统显微血管减压术,可减少手术用时及康复用时,改善三叉神经周边部分脑池容积,且并发症发生率较低,安全性较高。Objective To investigate the clinical comparative study of modified micro bone complete microvascular decompression and traditional microvascular decompression in the treatment of primary trigeminal neuralgia(ITN). Methods 82 patients with ITN from August 2014 to January 2016 in Anyang People's Hospital were randomly divided into groups,each with 41 cases.The control group was treated with traditional microvascular decompression,and the study group was treated with modified micro bone complete microvascular decompression.The operation condition,clinical effect and complication rate of the two groups were compared and analyzed.The volume changes of the peripheral nerve volume around the trigeminal nerve were analyzed between the two groups before and after operation. Results The operation time and length of stay in the study group were less than those in the control group,the difference was statistically significant ( P 〈0.05).The effective rate of the treatment group was 95.12% (39/41),higher than that of the control group 78.05% (32/41),the difference was statistically significant ( P 〈0.05).There was no significant difference between the two groups in the volume of the cerebral cistern around the trigeminal nerve before operation( P 〉0.05).The volume of cerebral cistern around the trigeminal nerve in two groups was higher than that before operation.The volume of cerebral cistern around the trigeminal nerve in the two groups was higher than that before operation( P 〈0.05).The complication rate of the study group was 12.20% (5/41),which was lower than that of the control group 31.72% (13/41),the difference was statistically significant ( P 〈0.05). Conclusion The modified micro bone complete microvascular decompression for primary trigeminal neuralgia is superior to traditional microvascular decompression.It can reduce operation time and recovery time,improve the volume of the cerebral cistern around the trigeminal nerve and the complication rate is low.The s
