脑室腹腔分流术后迟发性颅内出血临床分析

摘　　要：目的探讨脑积水脑室腹腔(VP)分流术后迟发性颅内出血的发生机制及相关危险因素分析。方法回顾性分析105例脑积水行VP分流术患者的临床资料。结果 105例患者中出现VP分流术后迟发性颅内出血16例,发生率15.24%(16/105);发生迟发性颅内出血与性别(P=0.207)、年龄(P=0.795)、原发病(P=0.207)、有无骨窗(P=0.731)及有无高血压史(P=0.206)无关。结论 VP分流术后迟发性颅内出血的发生与性别、年龄、原发病、有无骨窗及有无高血压史无关,术后早期重复头颅CT扫描有助于早期发现VP分流术后颅内出血,有利于改善患者预后。Objective To investigate the mechanisms and analysis related risk factors of delayed intracerebral hemorrhage secondary to ventricular cannulation during shunting procedures. Methods A retrospective analysis was used in 105 clinical data of hydrocephalus who underwent a VP shunt.Postoperative head computerized tomography scans were obtained repeatedly in every case of shunt surgery performed,according to the patient＇s condition or conventional performed within a week. Results The rate of delayed intracerebral hematoma or intraventricular hemorrhage after VP shunt placement was documented by routine neuroradiological follow up to be 15.24%（ 16 vs 105）.The occurrence of delayed intracranial hemorrhage with sex （ P =0.207）,the age （ P =0.795）,the primary disease （ P =0.207）,skull defect （ P =0.731） and the presence of hypertension （ P =0.206） has nothing to do. Conclusion The patient＇s gender,age,primary disease,independent of whether the skull defect and the presence of hypertension were not correlated with delayed intracerebral hemorrhage secondary to VP Shunt,early postoperative repeat head CT scans contribute to early detection intracranial hemorrhage secondary to VP shunt,will help to improve the prognosis of patients.
