AngioJet抽吸在布加综合征合并血栓中的初步应用

摘　　要：目的探讨Angio Jet血栓清除装置在布加综合征（BCS）并血栓形成患者中的临床应用价值。方法回顾性分析2016年6月至2016年12月行Angio Jet血栓抽吸、置管溶栓、球囊扩张、支架置入等综合介入治疗的3例BCS并血栓形成患者的临床资料和疗效。结果手术均成功,2例行Angio Jet血栓抽吸联合置管溶栓治疗,1例行单纯血栓抽吸,3例均行球囊扩张,2例置入下腔静脉支架。术后3、6、8个月彩色多普勒超声或CT静脉造影（CTV）随访,肝静脉、下腔静脉血流通畅,无严重并发症或死亡。2例血栓抽吸后出现一过性血红蛋白尿,经对症处理后好转。结论 Angio Jet血栓抽吸联合置管溶栓、球囊扩张、支架置入等综合介入治疗BCS并血栓形成微创、安全、有效,为临床提供了一种新的治疗选择。Objective To investigate the clinical application value of Angiojet thrombectomy device in patients of Budd Chiari syndrome complicated with thrombosis. Methods A retrospective analysis of 3 cases of Budd Chiari syndrome complicated with thrombosis,from June 2016 to December 2016,through Angiojet thrombus aspiration,catheter thrombolysis,balloon dilatation,stent implantation and some other combined interventional treatment,then analyse clinical data and curative effect. Results All the operations were successful. 2 cases received Angiojet thrombus aspiration combined with catheter thrombolysis,1 case received simple thrombus aspiration,3 cases underwent balloon dilatation,and 2 cases were implanted with inferior vena cava stent. After 3,6 and 8 months color Doppler ultrasound or CTV follow-up,the hepatic vein and inferior vena cava fluent blood flow,and no serious complications or death. Transient hemoglobinuria occurred after thrombus aspiration in 2 cases,and disappeared after symptomatic treatment. Conclusion Angiojet thrombus aspiration combined with catheter thrombolysis,balloon dilatation and stent implantation and some other combined interventional therapy in the treatment of patients of Budd Chiari syndrome complicated with thrombosis is a minimally invasive,safe and effective method,it provides a new treatment option for the clinic.
