摘　　要：目的提高对肾透明细胞肉瘤的认识,为该病的诊断、鉴别诊断和治疗提供依据。方法回顾性分析1例肾透明细胞肉瘤伴骨转移的诊疗经过,结合文献进行复习。结果患儿,男,7岁4个月,因"发现右肾包块4月余,血尿2周"来我院就诊,术前行CT、B超检查考虑右肾占位,遂行根治性右肾切除术,病理检查结果为右肾透明细胞肉瘤。术后行化疗,2个月后死于全身骨转移。结论肾透明细胞肉瘤恶性程度高,发现时已属于晚期,预后极差。因该病的治疗方案及不良预后均有别于其它相似疾病(如肾母细胞瘤等),所以正确的诊断和分期、良好的医患沟通、细致的手术操作及合理的术后治疗对于该肿瘤均有重要意义。Objective To improve the understanding of clear cell sarcoma of kidney and provide basis for diagnosis, differential diagnosis and treatment. Methods A case of renal clear cell sarcoma with bone metastasis was analyzed retrospectively.Results The patient was admitted to our hospital for he was found the right kidney mass for more than four months and hematuria for two weeks. Before operation, the right renal mass was taken into account by B-ultrasound and CT examination, so radical right nephrectomy was determined. The results showed clear cell sarcoma of kidney. Chemotherapy was performed after operation, but he died of bone metastasis two months later.Conclusions Renal clear cell sarcoma has a high malignant degree, and its prognosis is very poor. Because the treatment plan and adverse prognosis of this disease are different from other similar diseases, such as Wilms′ tumor, correct diagnosis and staging, good communication between doctors and patients, meticulous operation and reasonable postoperative treatment are all important for this tumor.
