黏膜保护剂保留灌肠预防宫颈癌放疗所致放射性直肠炎临床观察

摘　　要：目的探讨黏膜保护剂保留灌肠预防宫颈癌放疗所致放射性直肠炎的临床疗效。方法 90例Ⅱb～Ⅳa期行根治性放疗的宫颈癌患者随机分为观察组(n=42)和对照组(n=48),2组放疗方法相同,观察组于放疗前1 h排空大小便,将生理盐水50 mL+思密达3 g+地塞米松5 mg混合成灌肠液与放疗同步保留灌肠。对照组于放疗前排空大小便后直肠局部不用药物干预。比较2组急、慢性放射性直肠炎发生率。结果观察组放疗第4、5周(急性)Ⅱ度及以上放射性直肠炎发生率均低于对照组,差异均有统计学意义(P均<0. 05)。观察组放疗结束后6、12个月(慢性)放射性直肠炎发生率均低于对照组,差异均有统计学意义(P均<0. 05)。结论放疗前采用黏膜保护剂保留灌肠可预防宫颈癌放疗所致放射性直肠炎,可降低直肠炎发生风险以及直肠损伤程度。Objective To investigate the clinical effect of retention enema with mucosal protective agent on radiation proetitis in the patients with cervical cancer. Methods Ninety- patients with grade Ⅱb～Ⅳa cervical cancer treated with radical radiotherapy were randomly divided into the observation group （ n = 42 ） and the control group （ n = 48 ）, all the patients underwent the same radiation therapy. The observation group was given with retention enema of smeeta and dexamethasone at one hours before radiotherapy, while the control group did not administered with retention enema. The acute and late radiation proetitis incidences were compared between the two groups. Results The degree Ⅱ or above of acute radiation proetitis incidences at the 4th and 5th week of radiotherapy in the observation group were significantly lower than those in the control group （ P 〈 0.05 ）. The late radiation proetitis incidences at 6 and 12 months after radiotherapy in the observation group were significantly lower than those in the control group （ P 〈 0.05 ）. Conclusion It is possible to prevent and reduce the radiation proetitis by using retention enema with nmeosal protective agent for patients with cervical cancer before radiotherapy.
