摘　　要：目的探讨腹腔镜直肠癌根治术手术时间的影响因素,为临床提供参考。方法纳入本院2015年6月至2017年6月收治的80例接受腹腔镜直肠癌根治术患者作为研究对象,根据手术时间分为手术顺利组(47例,手术时间<120 min)和手术延迟组(33例,手术时间≥120 min)。采用Logistic多因素分析手术用时的影响因素。结果 80例患者中72例达到肿瘤完整切除,切缘阴性率为90%,多因素分析结果显示肿瘤直径(OR=2.492,95%CI=1.451~30.795,P=0.000)、肿瘤形态(OR=1.912,95%CI=1.062~14.486,P=0.003)及纤维化程度(OR=1.697,95%CI=2.093~26.307,P=0.000)是影响手术时间的独立因素。结论腹腔镜直肠癌根治术具有较高的肿瘤切除率,肿瘤直径、肿瘤形态及纤维化程度是影响腹腔镜直肠癌根治术手术时间的独立因素。Objective To analyze the explanatory factors for operation duration of laparoscopic radical resection of rectal cancer.Methods 80 patients with rectal cancer who received laparoscopic radical resection of rectal cancer in our hospital from June 2015 to June 2017 were selected as the research subjects. Patients were divided into two groups according to the duration of operation： uneventful operation group（47 patients, operation duration 120 min） and prolonged operation group（33 patients, operation duration ≥120 min）. Explanatory factors for operation duration were analyzed using multivariate logistic regression. Results Completed resection of the tumor was achieved in 72 out of 80 patients. Negative resection margin was found in 90% of the patients. Multivariate logistic regression showed that tumor size（OR = 2.492, 95%CI： 1.451~30.795, P = 0.000）, tumor morphology（OR = 1.912, 95%CI： 1.062~14.486, P = 0.003）, and degree of fibrosis（OR = 1.697, 95%CI： 2.093~26.307, P = 0.000） were independent explanatory factors influencing operation duration. Conclusion The laparoscopic radical resection of rectal cancer has a high successful rate of tumor resection. The diameter, the size of the tumor and the degree of fibrosis are independent explanatory factors that affect the operation duration of laparoscopic radical resection of rectal cancer.
