首发精神分裂症药物治疗疗效评价及对患者糖脂代谢安全性的影响

摘　　要：目的:研究齐拉西酮与奥氮平治疗首发精神分裂症患者的临床疗效,以及对其糖脂代谢安全性的影响。方法:选取2016年5月-2017年5月本院收治的首发精神分裂症患者106例,按照治疗方式不同将其分为甲组与乙组,各53例。甲组应用奥氮平治疗,乙组应用齐拉西酮治疗。比较两组临床效果,治疗前后的临床症状改善(PANSS、CGI-S评分)及糖脂代谢情况(FBG、LDL-C、HDL-C、TC、TG)。结果:甲组临床治疗总有效率为88.68%,低于乙组的92.45%,但比较差异无统计学意义(P>0.05);治疗前,两组PANSS与CGI-S评分比较,差异均无统计学意义(P>0.05);治疗后,两组PANSS与CGI-S评分均低于治疗前(P<0.05),但两组比较差异均无统计学意义(P>0.05);治疗后,两组血糖、血脂指标水平均优于治疗前,且乙组FBG、TG、LDL-C及TC水平均低于甲组,HDL-C水平高于甲组,比较差异均有统计学意义(P<0.05)。结论:对首发精神分裂症患者应用齐拉西酮与奥氮平进行治疗,均可获得良好的治疗效果,均可改善患者的PANSS与CGI-S评分,但是齐拉西酮更能维持患者机体血糖、血脂代谢的稳定性与安全性,临床应用价值较奥氮平更高。Objective：To study the clinical efficacy of Ziprasidone and Olanzapine in treatment of firstepisode schizophrenia and the safety of glycolipid metabolism.Method：A total of 106 patients with first-episode schizophrenia admitted to our hospital from May 2016 to May 2017 were selected,according to different treatment methods,they were divided into A group and B group,53 cases in each group.A group was treated with Olanzapine and B group was treated with Ziprasidone.The clinical effects of two groups were compared,the improvement of clinical symptoms（PANSS,CGI-S score） and glycolipid metabolism（FBG,LDL-C,HDL-C,TC,TG）before and after treatment were observed.Result：The total effective rate of A group was 88.68%,which was lower than 92.45% of B group,but the difference was not statistically significant（P〉0.05）.Before treatment,the PANSS and CGI-S scores of two groups were compared,the differences were not statistically significant（P〉0.05）, after treatment,the scores of PANSS and CGI-S in two groups were lower than those of before treatment（P〈0.05）, but between two groups were compared,the differences were not statistically significant（P〉0.05）.After treatment,the levels of blood lipids and blood glucose in both groups were better than those of before treatment,and the levels of FBG,TG,LDL-C,TC in B group were lower than those of A group,and the level of HDL-C was higher than that of A group,the differences were statistically significant（P〈0.05）.Conclusion：Both Ziprasidone and Olanzapine can be used to treat patients with first-episode schizophrenia,and both of them can improve the PANSS and CGI-S score,but Ziprasidone is more effective in patients body blood glucose,blood lipid metabolism stability and safety,clinical application of higher value than Olanzapine.
