健脾补肾方对于胃癌术后复发转移率、血清MMP-3及MMP-9水平的影响

摘　　要：目的观察健脾补肾方对于胃癌术后复发转移率及血清金属基质蛋白酶-3(MMP-3)、MMP-9水平的影响。方法将90例胃癌术后患者随机分为2组,其中对照组45例给予奥沙利铂+亚叶酸钙+5-氟尿嘧啶治疗,观察组45例在此基础上加用健脾补肾方治疗,观察2组治疗前后症状体征积分、Karnofsky评分(KPS)、血清MMP-3及MMP-9水平变化情况,统计2组临床疗效,随访复发转移率及不良反应发生率。结果 2组治疗后肢冷畏寒、舌淡胖或润、胃脘隐痛、便溏或夜尿增多及脉沉或迟评分,MMP-3和MMP-9水平均显著降低(P均<0. 05),且观察组治疗后以上指标均显著优于对照组(P均<0. 05);对照组治疗前后KPS评分比较差异无统计学意义(P> 0. 05);观察组治疗后KPS评分显著升高(P <0. 05),且观察组治疗后KPS评分显著高于对照组(P <0. 05);观察组治疗总有效率显著高于对照组(P <0. 05),不良反应发生率和随访复发率显著低于对照组(P <0. 05)。结论健脾补肾方用于胃癌术后患者可有效减轻相关症状体征,提高日常生活质量,下调MMP-3和MMP-9水平,降低远期复发风险,且有助于预防药物毒副作用。Objective It is to observe the effect of Jianpi Bushen Recipe on the recurrence and metastasis rate of gastric cancer after operation and the levels of serum MMP-3 and MMP-9.Methods Ninety cases of postoperative patients with gastric cancer were randomly divided into 2 groups, 45 patients in the control group were treated with oxaliplatin and calcium folate and 5-fluorouracil, 45 cases in the observation group were treated with Jianpi Bushen Recipe based on the control group. The changes of symptom and sign scores, Karnofsky score （KPS）, serum MMP-3 and MMP-9 levels were observed before and after treatment. The clinical curative effect, follow-up recurrence and metastasis rate, and incidence of adverse reactions were compared between the two groups.Results After treatment, the scores of cold limbs and chilly, tongue pale and fat, epigastric dull pain, loose stool or nocturnal urine increase, pulse sedimentation or late, and the levels of MMP-3 and MMP-9 were decreased significantly （all P 〈0.05）, and the above indexes in the observation group were significantly better than those in the control group （all P 〈0.05）. There was no significant difference in KPS score before and after treatment in the two groups （ P 〉0.05）, but the KPS score of the observation group after treatment was significantly higher than before treatment and the control group （all P 〈0.05）. The total effective rate of the observation group was significantly higher than that of the control group （ P 〈0.05）, the adverse reactions incidence and recurrence rate were significantly lower than those in the control group （all P 〈0.05）.Conclusion Jianpi Bushen Recipe can effectively relieve symptoms and signs in patients with gastric cancer after operation, improve the quality of daily life, decrease the levels of MMP-3 and MMP-9, reduce the risk of long-term recurrence. It also helps to prevent side effects of drugs.
