不同辅助方法治疗肌层浸润性膀胱癌保留膀胱术后效果观察

摘　　要：目的:探讨肌层浸润性膀胱癌保留膀胱术后采用不同辅助治疗的效果。方法:选择在笔者所在医院行保留膀胱术的90例肌层浸润性膀胱癌患者为研究对象,应用数字表法将其随机分为三组,每组30例,联合组术后实施动脉灌注化疗联合膀胱灌注化疗,动灌组术后仅实施动脉灌注化疗,膀灌组术后仅实施膀胱灌注化疗,比较三组的术后3年复发率、死亡率及生存质量评分。结果:术后3年内,联合组的复发率、死亡率均低于动灌组和膀灌组(P<0.05),而动灌组与膀灌组之间比较差异无统计学意义(P>0.05)。术后3年内,联合组存活患者的生存质量评分高于动灌组、膀灌组(P<0.05),而动灌组存活患者与膀灌组存活患者的生存质量评分比较差异无统计学意义(P>0.05)。结论:肌层浸润性膀胱癌患者保留膀胱手术后实施动脉灌注化疗联合膀胱灌注化疗,可有效减少复发率和死亡率,提高患者生存质量。Objective：To investigate the effect of different adjuvant therapy after bladder preserving operation for myometrial invasive bladder cancer.Method：90 patients with myometrial invasive bladder cancer were selected in our hospital.The patients were randomly divided into 3 groups,30 cases in each group.The combined group was treated with arterial perfusion chemotherapy combined with intravesical chemotherapy after operation.The dynamic irrigation group was carried out only arterial perfusion chemotherapy after operation,and the bladder irrigation group was carried out only bladder irrigation after bladder irrigation.After chemotherapy,recurrence rate,mortality and quality of life score of 3 groups were compared between the three groups.Result：Within 3 years after the operation,the recurrence rate and mortality rate of the combined group were lower than that of the dynamic irrigation group and the bladder irrigation group（P〈0.05）,but there was no significant difference between the dynamic irrigation group and the bladder irrigation group（P〈0.05）.Within 3 years after the operation,the survival quality score of the patients in the combined group was higher than that of the dynamic irrigation group and the bladder irrigation group（P〈0.05）,but there was no significant difference in the quality of life score between the surviving patients and the surviving patients in the bladder irrigation group（P〈0.05）.Conclusion：The patients with myometrial invasive bladder cancer can reduce the recurrence rate,mortality and improve the quality of life by using perfusion chemotherapy combined with intravesical chemotherapy after bladder operation.
