糖尿病肾病患者血清Leptin、Vaspin含量与肾功能、氧化应激及炎症反应的相关性

摘　　要：目的:探讨糖尿病肾病(DN)患者血清瘦素(Leptin)、内脏脂肪特异性丝氨酸蛋白酶抑制剂(Vaspin)含量与肾功能、氧化应激及炎症反应的相关性。方法:109例单纯2型糖尿病患者、76例糖尿病肾病患者分别作为单纯糖尿病组、DN组,在本院进行体检的健康志愿者100例作为正常对照组。对比三组研究对象血清中Leptin、Vaspin含量以及肾功能指标、氧化应激指标、炎症介质水平的差异,进一步分析DN患者血清中Leptin、Vaspin含量与肾功能损伤、氧化应激及炎症反应的内在联系。结果:单纯糖尿病组、DN组血清中Leptin、Vaspin的含量高于正常对照组,且DN组血清中Leptin、Vaspin的含量更高。DN组外周血中肾功能指标血肌酐(SCr)、尿素氮(BUN)、β_2微球蛋白(β_2-MG)、胱抑素-C(Cys-C)的水平高于单纯糖尿病组、正常对照组;氧化应激指标活性氧(ROS)、活性氮(RNS)的含量高于单纯糖尿病组、正常对照组,总抗氧化能力(T-AOC)的含量低于单纯糖尿病组、正常对照组;炎症介质转化生长因子_β(TGF-β)、白介素-1β(IL-1β)、白介素-18(IL-18)、单核细胞趋化蛋白-1(MCP-1)、细胞间黏附分子-1(ICAM-1)的含量高于单纯糖尿病组、正常对照组。相关性分析发现,DN患者血清Leptin、Vaspin含量与患者肾功能损伤程度、氧化应激及炎症反应程度均呈正相关。结论:DN患者存在血清Leptin、Vaspin含量异常上升,具体含量可客观反映机体肾功能损伤程度以及炎症应激反应程度。Objective：To investigate the correlation of serum Leptin and Vaspin contents with renal function,oxidative stress and inflammatory response in patients with diabetic nephropathy（DN）.Methods：A total of 109patients with type 2diabetes mellitus alone and 76patients with diabetic nephropathy were selected as diabetes group and DN group,respectively.Moreover,100healthy volunteers who underwent physical examination in our hospital were selected as normal control group.The differences in serum Leptin and Vaspin contents as well as renal function index,oxidative stress index and inflammatory mediator levels were compared among the three groups,and the internal correlation of serum Leptin and Vaspin contents in DN patients with renal function injury,oxidative stress and inflammatory reaction was further analyzed.Results：Serum Leptin and Vaspin contents of diabetes group and DN group were higher than those of normal control group,and serum Leptin and Vaspin contents of DN group were higher.Peripheral blood renal function indexes serum creatinine（SCr）,blood urea nitrogen（BUN）,β2 microglobulin（β2-MG）and cystatin-C（Cys-C）levels of DN group were higher than those of diabetes group and normal control group;oxidative stress indexes reactive oxygen species（ROS）and reactive nitrogen species（RNS）contents were higher than those of diabetes group and normal control group whereas total antioxidant capacity（T-AOC）content was lower than that of diabetes group and normal control group;inflammatory mediators transforming growth factor-β（TGF-β）,interleukin-1β（IL-1β）,interleukin-18（IL-18）,monocyte chemoattractant protein-1（MCP-1）and intercellular adhesion molecules-1（ICAM-1）contents were higher than those of diabetes group and normal control group.Correlation analysis showed that serum Leptin and Vaspin contents in DN patients were positively correlated with the degree of renal function injury,oxidative stress and inflammatory response.Conclusion：Serum Leptin and Vaspin conte
