摘  要:目的:探讨甲状腺乳头状微小癌中微淋巴管密度(MLD)和微血管密度(MVD)的检测及意义。方法:收集60例甲状腺乳头状微小癌标本及20例结节性甲状腺肿组织标本,采用免疫组织化学Envision两步法检测两种标本中D2-40和CD31表达情况,分别定量计数D2-40阳性表达的MLD和CD31阳性表达的MVD,分析它们与甲状腺乳头状微小癌转移的关系,并分析甲状腺乳头状微小癌中MLD与MVD的相关性。结果:甲状腺乳头状微小癌中MLD与MVD值高于结节性甲状腺肿,差异有统计学意义(P <0. 05);有淋巴结转移的甲状腺乳头状微小癌标本MLD与MVD值明显高于无转移淋巴结标本,差异有统计学意义(P <0. 05);在甲状腺乳头状微小癌中,MLD与MVD呈正相关关系(r=0. 742,P <0. 05)。结论:检测D2-40和CD31标记的MLD和MVD,对预测肿瘤淋巴结转移有潜在应用价值。Objective:To investigate the detection and significance of MLD and MVD in papillary thyroid micro-carcinoma. Methods: 60 specimens of papillary thyroid micro-carcinoma and 20 specimens of nodular goiter tissues were collected. The expression of D2-40 and CD31 in the two kinds of specimens was detected by immunohistochemistry. The micro-lymphatic vessel density (MLD) and micro-vascular density (MVD) ,marked by D2-40 and CD31,in papillary thyroid micro-carcinoma and nodular goiter tissues were quantitatively counted.Also their correlation with lymph node metastasis in papillary thyroid micro-carcinoma were analyzed. Results: The values of MLD and MVD in papillary thyroid micro-carcinoma were higher than those in nodular goiter tissues, and the difference was statistically significant ( P〈 0.05).The values of MLD and MVD in thyroid papillary microcarcinoma with lymph node metastasis were significantly higher than those without lymph node metastasis, and the difference was statistically significant( P 〈0.05). There was a positive correlation between MLD and MVD in papillary thyroid carcinoma ( r=0.742,P 〈0. 05). Conclusion: The detection of MLD and MVD marked by D2-40 and CD31 has potential application in predicting lymph node metastasis.
