摘  要:目的:探讨Freedom能谱CT成像对小肝癌及转移性肝癌的早期诊断的临床应用价值。方法:入选30例临床疑诊为小肝癌患者或已确诊为其他部位肿瘤,进行肝脏增强CT扫描检测转移瘤者,行能谱CT扫描并检测肿瘤病灶的CT值、碘含量及病灶检出率等数据。结果:碘基图像下原发性肝癌与转移瘤三期碘浓度均无统计学差异(P>0.05)。原发性肝癌、肝转移瘤与肝血管瘤的碘浓度在动脉期有明显统计学差异(P<0.05),在门脉期、延迟期的碘浓度三者无明显统计学差异(P>0.05)。单能量图像在病灶检出、成像质量等方面与混合能量图像相比,有明显差异,具有统计学意义。结论:Freedom能谱CT成像在小肝癌及转移性肝癌的诊断检出率较常规增强CT扫描高,对临床早期发现小肝癌及肝转移瘤有一定的临床价值。Objective: To investigate the clinical value of early diagnosis by Freedom spectrum CT imaging for small hepatocellular carcinoma and metastatic liver cancer. Methods: 30 cases in our hospital and clinically suspected for small hepatocellular carcinoma or those have been confrmed for other parts of the tumor, Using GSI imaging scanning mode, The CT value, iodine content and lesion detection rate were detected. Result: There were no signifcant diferences in the three phase of iodine based images of primary liver cancer and metastatic tumor (P〉0.05). The iodine concentration of primary hepatic carcinoma, hepatic metastasis and hepatic hemangioma were signifcantly diferent (P〈0.05). There was no statistically signifcant diference in the concentration of iodine in the portal vein phase and the delayed phase (P〉0.05). Compared with QC images, Mono image has signifcant diference in the detection and imaging quality of lesions. Conclusion: The Spectrum CT has advantages in the analysis of the tumor homology and early diagnosis of small liver tumors.
