刺络放血、针刺联合平胃散综合治疗中风后顽固性呃逆疗效观察

摘　　要：目的观察刺络放血、针刺联合平胃散综合治疗中风后顽固性呃逆的临床疗效。方法选取2015年3月至2016年8月期间收治的120例中风后顽固性呃逆患者为研究对象,根据随机数表法分为两组,每组60例。所有患者均接受中风常规治疗,在此基础上,对照组采用平胃散治疗,研究组采用刺络放血、针刺联合平胃散综合治疗。比较两组治疗前后神经功能缺损情况、临床症状及各时段起效率,并评价两组临床疗效及安全性。结果与本组治疗前比较,两组NIHSS评分、中医证候积分在治疗后均明显降低,且研究组下降幅度更大,差异具有统计学意义(P<0.05);研究组<0.5 h、0.5~4 h时段起效率较对照组更高,而>4 h时段起效率较对照组更低,差异具有统计学意义(P<0.05);研究组临床疗效总有效率高于对照组,差异具有统计学意义(P<0.05);治疗期间,两组均未出现不良反应及肝肾功能异常。结论采用刺络放血、针刺联合平胃散综合治疗中风后顽固性呃逆,可显著提高临床疗效,缓解临床症状,改善患者神经功能,促进患者身体恢复,优势显著,值得大力推广。Objective To observe the clinical efficacy of bloodletting,acupuncture combined with Pingwei powder in treatment of post-stroke intractable hiccup. Methods A total of 120 patients with post-stroke intractable hiccup in our hospital from March 2015 to August 2016 were divided into two groups by random number table,60 cases in each. All patients received routine stroke treatment,on this basis,control group took Pingwei powder and research group took bloodletting,acupuncture combined with Pingwei powder. The neurological deficit,clinical symptoms and effective rate in each period of two groups before and after treatment were compared,the clinical efficacy and safety of two groups were evaluated. Results Compared with before treatment,the NIHSS score,TCM symptom score of two groups significantly decreased after treatment,and the descend range of research group was larger（P〈0.05）; The effective rate in 0. 5 h and 0. 5 ~ 4 h of research group was higher than that of control group,and the effective rate in 〉4 h of research group was lower than that of control group（P〈0.05）; The total effective rate of research group was 90. 00%（54/60）,which was higher than71. 67%（43/60） of control group（P〈0.05）; During treatment,there was no adverse reactions and liver-kidney dysfunction in two groups. Conclusion Bloodletting,acupuncture combined with Pingwei powder for treating post-stroke intractable hiccup can significantly increase clinical efficacy,relieve clinical symptoms,improve nerve function and promote patients＇ recovery,which has significant advantages and is worthy of wide promotion.
