芪参还五胶囊联合羟乙基淀粉130/0.4对后循环缺血性眩晕患者的疗效及血流变、血动力学指标的影响

摘　　要：目的研究芪参还五胶囊联合羟乙基淀粉130/0.4对后循环缺血性眩晕(PCIV)的疗效及对血流变、血动力学指标的影响。方法选取2015年3月至2016年8月收治的PCIV患者120例,根据随机数字表法分为观察组及对照组各60例。对照组患者给予常规治疗及羟乙基淀粉130/0.4治疗,观察组则在对照组治疗的基础上加用芪参还五胶囊治疗。分别比较两组临床疗效、治疗前后眩晕症状、血液流变学指标以及血流动力学指标变化情况。结果观察组治疗后总有效率与对照组比较差异具有统计学意义(P<0.05)。治疗后两组患者的DARS评分均低于本组治疗前,而观察组治疗后又低于对照组,差异具有统计学意义(P<0.05)。治疗后观察组血浆黏度、全血高切还原黏度、全血低切还原黏度水平均低于对照组,差异具有统计学意义(P<0.05)。治疗后观察组左侧椎动脉VM、右侧椎动脉VM、基底动脉VM均高于对照组,具有显著性差异(P<0.05)。结论芪参还五胶囊联合羟乙基淀粉130/0.4液治疗PCIV的疗效显著,可有效改善患者的血流变及血动力学指标。Objective To study the curative effect of Qishen Huanwu capsule combined hydroxyethyl starch 130/0. 4 on patients with posterior circulation ischemic vertigo（PCIV） and the effect on blood dynamics and hemorrheology indexes. Methods 120 cases of patients with PCIV in our hospital from March 2015 to August 2015 were selected and divided into observation group and control group according to random number table method,with 60 cases in each group. The control group was given conventional therapy and hydroxyethyl starch 130/0. 4. On the basis of the control group,the observation group was added Qishen Huanwu capsule. The clinical curative effect,vertigo symptom,hemorheology indexes before and after treatment as well as changes in hemodynamic parameters of two groups were compared. Results The total effective rate of observation group was 96. 67%（58/60）,and there was significant difference compared with 85. 00%（51/60）of the control group with（P〈0.05）. DARS rating of two groups of patients after treatment was lower than before treatment,and the observation group was lower than the control group with significant differences（all P〈0.05）. The blood plasma viscosity,whole blood high shear reductive viscosity,whole blood low shear reduction viscosity levels of the observation group after treatment were lower than the control group,with significant difference（P〈0.05）. After treatment group,the left side of the vertebral artery VM,the right side of the vertebral artery VM,basal artery VM of observation group were higher than control group,with significant difference（P〈0.05）. Conclusion Qishen Huanwu capsule with hydroxyethyl starch 130/0. 4 fluid therapy in the treatment of PCIV has remarkable efficacy,which can effectively improve hemorrheology and blood dynamics index of patients.
