上颌单颗前牙即刻种植修复与延期种植修复对患者植体周围软组织的近期临床观察

摘　　要：目的探讨上颌单颗前牙即刻种植修复与延期种植修复对患者植体周围软组织的近期临床疗效。方法选择2015年8月—2016年10月在本院进行上颌单颗前牙种植修复的患者47例,根据种植方案不同分为对照组23例和观察组24例。对照组采用延期种植修复,观察组采用即刻种植修复。对所有患者随访1年,比较两组种植成功率;分别于术前及术后12个月测量牙槽嵴顶垂直向吸收高度,比较两组牙槽骨吸收量;采用红色美学指数(PES)评价种植义齿周围软组织美学效果,比较两组软组织塑形情况;采用VAS评分法评价患者对周围软组织色泽、咀嚼功能、附着高度、整体美观的满意程度,比较两组主观满意度。结果两组种植修复1年后种植体均骨性结合,无松动、脱落的现象,成功率均为100%;观察组种植1年后牙槽骨吸收量低于对照组(P<0.05);观察组与对照组修复1年后PES评分均高于修复后即刻(P<0.05);观察组修复后即刻与修复1年后种植义齿周围软组织美学效果均优于对照组(P<0.05);观察组对周围软组织色泽、咀嚼功能、附着高度、整体美观等方面满意度均高于对照组(P<0.05)。结论上颌单颗前牙即刻种植修复对患者植体周围软组织的近期疗效优于延期种植修复,具有疗程短、创伤低、成功率高、牙槽骨吸收量低、软组织塑形效果好、患者满意度高等优点,值得推广应用。Objective To investigate the short-term clinical efficacy of immediate implant restoration and delayed implant repair of maxillary single anterior teeth on the patient＇s surrounding soft tissue.Methods A total of 47 patients with maxillary single anterior teeth implanted in our hospital from August 2015 to October 2016 were selected. According to the different implantation schedules,they were divided into control group（ 23 cases）and observation group（ 24 cases）. The control group was treated with delayed planting and the observation group was immediately implanted. All patients were followed up for 1 year. The success rate of the two groups was compared. The vertical absorption height of the alveolar ridge was measured before surgery and 12 months after surgery. The alveolar bone re-sorption was compared between the two groups; the red aesthetic index（ PES） was used to evaluate the esthetic effect of the soft tissues around implant denture,and the soft tissue shape of the two groups was evaluated. The VAS score was used to evaluate the patients ＇ satisfaction with the color of the surrounding soft tissue,chewing function,attachment height and overall appearance. The subjective satisfaction was compared between the two groups.Results After one year of implantation,the implants were bony union,no loosening and shedding,and the success rate was 100%. The alveolar bone absorption in the observation group was lower than that of the control group after one year of planting（ P 〈 0.05）. PES scores of the observation group and the control group were higher than those of the repair immediately after the first year of repair（ P 〈 0. 05）. The aesthetic effects of the soft tissue around the implant dentures immediately after the repair and after the repair of the observation group were better than the control group（ P 〈 0.05）.
