摘  要:目的探讨伴有轴性超高度近视的白内障患者超声乳化术后屈光误差的影响因素。方法回顾性分析行超声乳化联合人工晶状体植入术,伴有眼轴〉30 mm的轴性超高度近视的白内障共50例(61只眼)的临床资料。测量患者术后屈光状态,并与预期屈光状态对比,计算平均绝对屈光误差(mean absolute refractive error,MAE)。结果手术均顺利。随访2~6个月,平均(3.14±0.95)月。目标屈光度为(-2.82±1.02)D,术后实际屈光度为(-2.14±1.30)D,两者差异有统计学意义(t=-6.350,P=0.000)。MAE为(0.88±0.52) D,MAE与眼轴长度正相关(r=0.549,P=0.000)。术后42只眼(68.9%)的屈光误差在±1.0 D以内,而负度数人工晶状体植入者中仅10只眼(43.5%)的屈光误差在±1.0 D内,与零度数及正度数人工晶状体植入相比,其MAE较大,差异有统计学意义(P=0.000)。结论伴有轴性超高度近视的白内障术后屈光误差与眼轴长度正相关,负度数人工晶状体植入的术后屈光误差更大。ObjectiveTo investigate the affecting factors of refraction error in cataract with extreme axial myopia after phacoemulsification. MethodsThe data of 61 eyes of 50 cases of cataract with axial length 〉30 mm extreme high axial myopia from Jan. 2017 to Dec. 2017 who received phacoemulsification were retrospectively analyzed. Postoperative refractions were measured and compared with predictive refractions. The mean absolute refractive error(MAE)was calculated. ResultsThe follow-up time was 2-6 months, with average of (3.14±0.95) months. The target spherical equivalent was (-2.82±1.02) D, postoperative spherical equivalent was (-2.14±1.30) D, the difference was significant between target and postoperative spherical equivalent(t=-6.350, P=0.000). MAE was (0.88±0.52) D. There was positive correlation between MAE and axial length(r=0.549, P=0.000). The refraction errors was within ±1.0 D of target spherical equivalent in 42 eyes(68.9%), however, only 10 eyes(43.5%)of minus-IOL showed a prediction error within ±1.0 D. Compared with plus-IOL and zero-IOL, MAE of minus-IOL was higher, the difference was statistically significant(P=0.000). ConclusionRefraction errors are increasing with axial length, which are more pronounced with minus-IOL.
