路优泰联合绝经激素治疗围绝经期抑郁症的效果分析

摘　　要：目的评价圣·约翰草提取物片(路优泰)联合绝经激素治疗(MHT)对治疗女性围绝经期抑郁症的疗效及不良反应发生率。方法女性围绝经期抑郁症患者120例,按入组先后顺序分为3组,分别为A组(MHT组)、B组(路优泰+MHT组)、C组(路优泰组)各,每组40例。对各组患者治疗前和治疗后4、8、12周分别进行汉密顿抑郁量表(HAMD)评分,根据HAMD评分下降率评价各组的疗效,统计各组不良反应发生率,比较3组不良反应发生率的差异。结果 3组女性围绝经期抑郁症患者治疗前年龄及HAMD评分比较差异无统计学意义(P>0.05)。治疗4、8、12周后,3组患者HAMD评分均较同组前一次HAMD评分下降,差异有统计学意义(P<0.05)。治疗12周后,B组与A组患者疗效的差异有统计学意义(P<0.05),B组与C组疗效差异有统计学意义(P<0.05),但A组与C组疗效差异无统计学意义(P>0.05),结果显示B组对女性围绝经期抑郁症疗效优于A组及C组。治疗12周末,3组患者不良反应发生率差异无统计学意义(P>0.05)。结论路优泰+绝经激素治疗对女性围绝经期抑郁症患者的疗效明显优于单用绝经激素治疗及路优泰治疗,其疗效快速、稳定,对治疗女性围绝经期抑郁症未明显增加药物不良反应发生率,服药依从性好,对临床应用有一定指导意义。Objective To evaluate of the effect and incidence of medical adverse events on St. John wort extract combined with menopause hormone therapy （MHT） in perimenopause depressive disorder. Methods 120 patients were divided into four groups ：40 pa- tients in group A （MHT） ,40 patients in group B （St. John wort extract combined MHT） ,40 patients in group C （St. John wort ex- tract） and all patients had complete all clinical data. All patients had an evaluation of Hamilton depression scale （HAMD） with prior treatment,and an evaluation of effect on HAMD scores of decrease and incidence of medical adverse events. Results There were no difference between three groups in HAMD scores and age before the intervene （P 〉 0.05 ） ;At the 4th,Sth, 12th in three groups HAMD scores was different comparing with themselves of previous HAMD scores as the course of treatment progress （ P 〈 0.05 ） ; At the 12th week of treatment,the effect of group A and group B was different （ P 〈 0.05 ）, the effect of group B and group C was different （ P 〈 0.05） ,there are no difference between group A and group C （P 〉 0.05 ）. The effect of group B in treatment of perimenopause depres- sion was superior than that in group A and group C ;At the 12th week of treatment,there was no difference between three groups of inci- dent of medical adverse events （P 〉 0.05 ）. Conclusion The effects of St. John wort extract combined menopause hormone therapy group in perimenopause depression has advantage than menopause hormone therapy group and St. John wort extract group, and the effect is fast, stable and lasting, and the rate of adverse reaction has no increase and has better compliance. It has certain guiding significance for clinical application.
