摘　　要：目的分析乳腺叶状肿瘤(PTB)的MRI特征,并与病理结果对照,提高对该病的诊断水平。方法回顾性分析12例经手术病理证实的乳腺叶状肿瘤的MRI表现,包括形态学、平扫信号、动态增强TIC形态、弥散信号及表观扩散系数(ADC值)特点。结果 12例PTB患者经术后病理诊断为良性6例,交界性3例,恶性3例。T1WI病灶呈等信号或低信号,其中3例夹杂斑片状高信号影,T2WI均为不均匀高信号,12例DWI均呈高信号,相应ADC值低于正常乳腺组织。动态增强病灶实质呈不均匀强化,12例时间-信号强度曲线(time-signal intensity curve,TIC)类型呈Ⅰ型3例,8例Ⅱ型,Ⅲ型1例。结论乳腺叶状肿瘤的MRI表现多样,MRI检查有助于乳腺叶状肿瘤的诊断,确诊仍需对整个病灶行组织病理学检查。Objective To analyze the MRI features of breast phyilodes tumor and to compare with the pathological results, in order to improve the diagnostic level of this disease. Methods The data of 12 patients with phyllodes tumor confirmed by operative pathology were analyzed retrospectively, which included morphology, plain scan signal, dynamic enhanced TIC morphology, diffusion-weighted signal and characteristics of apparent diffusion coefficient （ADC value）. Results Of 12 cases, 6 cases were diagnosed as benign, borderline in 3 cases and malignant in 3 cases by postoperative pathology. T1WI lesions showed ISO signal or low signal, including 3 cases with patch like high signal intensity. T2WI showed heterogeneous high signal. DWI of 12 cases all showed high signal, and the corresponding ADC value was lower than that of normal breast tissue. The dynamic enhanced lesions showed heterogeneous enhancement. The time-intensity curve （TIC） of 12 cases included 3 in type Ⅰ, 8 in type Ⅱ and 1 in type Ⅲ. Conclusion MRI findings of breast phyllodes tumor are various. MRI examination is helpful for diagnosis of phyllodes tumor of the breast. Histopathological examination of the whole lesion is still needed for making a definite diagnosis.
