摘 要:目的:评价临床3种常用充填材料在釉牙骨质界龋损治疗中的疗效。方法:将541个近龈乳头釉牙骨质界龋损患牙按照窝洞位置与釉牙骨质界的关系分3类,每类再随机分为3个处理组,超声去腐后分别用银粉玻璃离子、Multifil GIX玻璃离子、P60光固化复合树脂充填,并进行6~24个月的随访,根据疗效评定标准对3种材料在釉牙骨质界龋损的疗效进行评价。结果:3组材料分别充填釉牙骨质界以上以及位于釉牙骨质界水平的窝洞时,充填成功率由高到低依次为银粉玻璃离子组(91. 59%)、Multifil GIX玻璃离子组(87. 09%)、P60光固化复合树脂组(74. 80%),组间两两比较均无统计学差异(P> 0. 05)。3组材料分别充填完全位于釉牙骨质界以下的窝洞时,充填成功率由高到低依次为银粉玻璃离子组(81. 25%)、Multifil GIX玻璃离子组(60. 34%)、P60光固化复合树脂组(30. 19%),且各组间两两比较均具有统计学差异(P <0. 05);充填材料的2年脱落率由低到高依次为银粉玻璃离子组(4. 2%)、Multifil GIX玻璃离子组(7. 7%)、P60光固化复合树脂组(15. 9%)。结论:与Multifil GIX玻璃离子和P60光固化复合树脂比较,银粉玻璃离子治疗近龈乳头釉牙骨质界龋损且窝洞位于釉牙骨质界以下的患牙2年成功率最高,可作为该类龋损充填的优选材料。AIM: To compare the effects of three different kinds of commonly used filling materials for the treatment of enamelo-cemental junction(ECJ) caries near gingival papillae. METHODS: Based on the location relationship between caries with the enamelo-cemental junction, gum excision, crown extension or flap operation was performed before the caries tissue was removed. Then the samples ( n =541) were randomly divided into 3 groups in each caries location category, and the caries tissue was removed using ultrasonic tips and caries-removing gel. The cavities were filled with silver-reinforced glass ionomer, Multifil GIX glass ionomer, and P60 light-cured composite resin, respectively. The patients were then followed up for 6-24 months. RESULTS: For the caries lesion at or above ECJ filling success rate of silver-reinforced glass ionomer group, Multifil GIX glass ionomer group and P60 light-cured composite resin group was 91.59%, 87.09% and 74.80% respectively (between each 2 groups, P 〉0.05); for the carie lesion below ECJ that was 81.25%,60.34% and 30.19% respectively(betwee each 2 groups, P 〈0.05). 2 years after treatment, the exfoli-ation reate of the filling materia of the 3 groups was 4.2%, 7.7% and 15.9% respectively.CONCLUSION: Silver-reinforced glass ionomer is more effective in the treatment of the enamelo-cemental junction caries near gingival papillae than Multifil GIX glass ionomer and P60 light-cured composite resin.
