窄蒂侧颌颈皮瓣在面部皮肤癌术后创面修复中的应用

摘　　要：目的探讨窄蒂侧颌颈皮瓣应用于面部皮肤癌术后创面的修复效果。方法设计耳前窄蒂的侧颌颈部皮瓣,修复面部皮肤癌术后组织缺损,皮瓣最大10 cm×6 cm,最小3. 5 cm×2. 5 cm,蒂部宽1. 5 cm～2. 0 cm,长2. 0 cm～5. 0 cm。结果应用窄蒂侧颌颈皮瓣修复组织缺损10例,2例皮瓣远端出现淤血,延期愈合,其余均一期愈合,外形满意。结论窄蒂侧颌颈皮瓣切取简便,供区位置隐蔽,且能直接闭合,蒂部旋转范围大,外形满意,同时能及早进行皮肤癌的综合治疗,是面部皮肤癌术后创面修复的理想选择。Objective To study the application of the narrow pedicle lateral maxillofacial flap in the reconstruction of the facial carcinoma patients. Methods The narrow pedicle lateral maxillofacial flap located in the anterior ear region was designed to repair the soft tissue defect after the facial carcinoma resection.The maximum size of the flap was 10 cm×6 cm and the minimum 3.5 cm×2.5 cm.The pedicle width was 1.5～2.0 cm and the length 2.0～5.0 cm. Results The narrow pedicle lateral maxillofacial flaps was applied in 10 cases.There was congestion in the distal part of the flaps with delayed healing in 2 cases,and the other cases healed well with satisfactory appearance. Conclusion The narrow pedicle lateral maxillofacial flap has prominent advantages,such as the straightforward harvesting procedure,the concealed donor site which can be closed directly,the flexible pedicle and the satisfactory surgical results.At the same time,the comprehensive treatment of skin carcinoma could be carried out early.It is an ideal choice for the repair of the wound after the facial carcinoma resection.
