环乳晕入路保留乳头乳晕乳腺癌改良根治术的应用效果

摘　　要：目的探讨环乳晕入路保留乳头乳晕乳腺癌改良根治术的应用效果。方法采用回顾性、随机、抽样、对照研究方法,选择118例乳腺癌早期患者,按治疗方法分为观察组、对照组,各59例。对照组进行常规乳腺癌标准根治术,观察组进行环乳晕入路保留乳头乳晕乳腺癌改良根治术,记录与随访2组预后。结果对照组术后12周并发症的发生率是22. 0%,显著高于观察组并发症的发生率8. 5%(P <0. 05)。术后12周观察组的生存质量各维度评分均显著高于对照组(P <0. 05)。随访1年后,观察组患者的局部复发率是5. 0%,远处转移率是8. 5%,生存率是91. 5%,与对照组的6. 8%、10. 2%、89. 8%相比,差异都无统计学意义(P> 0. 05)。结论环乳晕入路保留乳头乳晕乳腺癌改良根治术的应用能减少对患者的创伤,改善乳房的美观,提高患者的生存质量,且不会影响患者的远期预后,可作为治疗早期乳腺癌的首选手术方法。Objective To evaluate the effects of modified radical mastectomy with annular areola approach. Methods Used a retrospective,randomized,controlled study,118 cases of early breast cancer patients were selected and divided into the observation group and the control group,according to the different treatment methods,each with 59 cases.The control group was given routine standard radical mastectomy treatment,the observation group was given the circum areola approach reserved for mastectomy nipple areola modified radical mastectomy,recorded and followed-up of patients＇ prognosis. Results The postoperative 12 weeks of incidences of complications such as skin flap,skin flap necrosis,infection and upper limb lymphedema was 8.5% in the observation group,so that was 22.0% in the control group,the observation group was significantly lower than the control group （ P〈0.05）.The postoperative 12 weeks of scores of body,social support,psychology and spirit of the observation group were significantly higher than those of the control group （ P〈0.05）.After 1 year of followed-up,the local recurrence rate,distant metastasis rate and survival rate in the observation group were 5.0%,8.5% and 91.5%,respectively,which were not statistically different compared with those of the control group （6.8%,10.2% and 89.8%） （ P〉0.05）. Conclusion The modified radical mastectomy with circum areola approach for breast cancer can reduce the nipple for patients with trauma,it can improve breast appearance and improve the quality of life of patients,and will not affect the long-term prognosis of patients,which is the preferred surgical method for treatment of early breast cancer.
