摘  要:近些年食管癌的发病率逐年上升,严重影响着人们的健康,而大部分早期食管癌在内镜下即可获得根治性治疗,5年生存率可超过95%。内镜下切除的绝对适应证为局限在上皮层或黏膜固有层的病灶。目前主要通过超声内镜判断浸润深度,尽管超声内镜是一项较为准确的技术,但对于通过超声内镜判定食管癌分期也曾一度受到怀疑,认为没有临床意义。也有报道显示,高频探头对于区分浸润至黏膜层或黏膜下层的准确率为75%~95%,但其受人为因素影响大。因此,目前急需一项新型的技术来提高其浸润深度判断的准确率,从而选择最优化的治疗方案。光学相干断层成像技术是一种新型的生物医学光学成像方式,它可以安全无创地提供具有深度层析信息的生物组织微解剖图像。此综述指出了食管癌目前的现状及光学相干断层成像技术的原理,归纳了光学相干断层成像技术对早期食管癌浸润深度的诊断意义。Recentl)3 with the increase of the morbidity in esophageal cancer, it becomes one of the most health threatening diseases in the world. Endoscopic resection is an optimal treatment for most cases of early esophageal cancer, with similar efficacy to surgery. The indication of endoscopic resection for early esophageal cancer is limited to carcinoma in situ and tumors invading the lamina propria. Now, tumor invasion in early esophageal cancer is mainly evaluated by endoscopic ultrasonography. It was once considered to be of no clinical significance for the tumor staging in early esophageal cancer, although endoscopic ultrasonography was an accurate technique. However, several studies reported that the accuracy of high-frequency endoscopic ultrasonography for tumor staging in early esophageal cancer was 75%-90%. Besides, it is likely to be influenced by human factors. Thus, a new technique is needed to improve the accuracy of tumor staging. Optical coherence tomography(OCT) is a noninvasive technology that can produce high-resolution cross-sectional images. This review described the epidemiology of esophageal cancer in China, and summarized the principle of optical coherence tomography and demonstrated the accuracy of OCT for evaluating the tumor invasion of early esophageal cancer.
