摘 要:甲状腺相关眼病(TAO)是成人最常见的眼眶疾病之一,属器官特异性自身免疫性疾病,目前致病机制尚不完全清楚。临床表现复杂多样,上睑提肌和Müller's肌的炎症、纤维化、挛缩,眼球突出,垂直眼外肌的挛缩等多种因素,导致患者上下眼睑退缩,这是轻、中度TAO患者最常见的临床表现。眼睑退缩轻微者可使TAO患者出现眼干、异物感等不适,严重者则可有畏光、流泪,甚至暴露性角膜病变,同时明显影响患者外观,对患者生理及心理均造成很大影响,影响生活质量。因此,眼睑退缩的处理在TAO的治疗过程中占重要地位。根据病情严重程度及疾病所处时期,针对TAO患者眼睑退缩的治疗时机、治疗措施的选择也有区别,观点也在日益更新和变化。Thyroid associated ophthalmopathy(TAO) is one of the most common orbital diseases in adults,and it is an organ-specific autoimmune disease,and the pathogenesis is not entirely clear. The clinical manifestations are complex and varied,such as the inflammation,fibrosis and contracture of the levator palpebrae superioris and Müller's muscles,exophthalmos,contracture of the vertical extraocular muscles and so on,resulting in upper and lower eyelid retraction,which are the most common clinical features of mild to moderate TAO. In mild cases,the eyes will experience dry,gritty. In more severe cases,patients suffer from photophobia,tears and even corneal exposure. At the same time,it has a great impact on patients' physiology,psychology and the quality of life due to their unacceptable appearance. Therefore,the dealwith of the eyelid retraction plays an important role in the treatment of TAO. According to the severity and the stage of the disease,there are also differences in the treatment timing and treatment options for the eyelid retraction of TAO patients,and the viewpoints are also being updated and changed.
