小野寺指数与直肠癌术后放化疗毒副反应相关性研究

摘　　要：目的:探讨直肠癌患者术后小野寺指数对辅助放化疗毒副反应的预测作用。方法:选取2014年1月—2017年12月接受辅助放化疗的Ⅱ、Ⅲ期直肠癌术后患者,其中营养良好组纳入32例(小野寺指数≥45),营养不良组纳入32例(小野寺指数<45)。比较两组患者的白细胞减少、血小板减少、腹泻、恶心、呕吐、食欲减退、疲劳、膀胱和尿道炎8种急性毒副反应。结果:营养不良组恶心(62.5%vs 34.4%,P=0.024)和腹泻(78.1%vs 46.9%,P=0.010)的发生率显著高于营养良好组,进一步分析重度毒副反应在两组的区别,发现营养不良组的重度恶心发生率显著高于营养良好组(12.5%vs 0,P=0.039)。结论:直肠癌患者术后小野寺指数对辅助放化疗的毒副反应具有较好的预测作用。Objective： To explore the association between Onodera prognostic nutrition index and adverse effects in postoperative rectal cancer patients who received adjuvant radiochemothera- py. Methods： 32 good-nourishment （Onodera index≥45） patients and 32 poor-nourishment（On- odera index 〈 45） were collected. The adverse effects including leukopenia, thrombocytopenia, diar- rhea, nausea, vomit, loss of appetite, fatigue, cystitis and urethritis were compared between good-nourishment group and poor-nourishment group. Results： The incidence of nausea（62.5% vs 34.4%, P=0.024） and diarrhea（78.1% vs 46.9%, P=0.010） were significantly higher in poor-nour- ishment group than in good-nourishment group. We further analyzed the difference of severe ad- verse effects between the two groups and found the incidence of severe nausea was higher inpoor-nourishment group than in good-nourishment group （12.5% vs 0%, P=0.039）. Co nclusi on： Our results show Onodera prognostic nutrition index has good predictive value for adverse effects in postoperative rectal cancer patients who received adjuvant radiochemotherapy.
