声辐射力脉冲成像技术评价消脂护肝胶囊治疗非酒精性脂肪性肝病临床疗效的价值

摘　　要：目的 :探讨声辐射力脉冲成像技术(ARFI)在评价消脂护肝胶囊治疗非酒精性脂肪性肝病临床疗效中的应用价值。方法:选取空腹血糖或血脂升高,除外病毒性肝炎、药物性肝病、全胃肠外营养、肝豆状核变性等可导致脂肪肝的特定疾病患者150例,分别行肝脏CT检查及超声检查,获得肝、脾CT值比值及声触诊组织定量(VTQ)值,记录2种检查方法检出轻、中、重度非酒精性脂肪肝例数,计算并比较2种方法对不同程度脂肪肝的检出率。其中经ARFI检出者采用消脂护肝胶囊治疗3个月后,再次对轻、中、重脂肪肝3组行声触诊组织定量(VTQ)值测定,对比每组患者治疗前后的VTQ值。结果:ARFI与肝脏CT检查对不同程度脂肪肝患者检出率比较差异无统计学意义(P> 0.05)。3组采用消脂护肝胶囊治疗前后VTQ比较,差异均有统计学意义(均P<0.05)。结论:ARFI可用于量化诊断非酒精性脂肪性肝病和评价消脂护肝胶囊的疗效。Objective：To investigate the application value of acoustic radiation force impulse （ARFI） technology in evaluating the therapeutic effect of Xiaozhihugan capsule in the treatment of nonalcoholic fatty liver disease. Methods：Select 150 cases of high blood glucose or hyperlipidemia hospitalized and outpatient patients in our hospital,who had no specific diseases,such as viral hepatitis,drug-induced liver disease,total parenteral nutrition,and hepatolenticular degeneration,which could lead to fatty liver. Give these patients CT examination and liver ultrasound examination,obtain the liver/spleen CT ratios and virtual touch tissue quantification （VTQ） values. Then use two methods to detect the number of mild,moderate and severe non-alcoholic fatty liver disease,calculate the detection rates of each group,and compare the detection rates of two methods in each group. After 3months’ treatment with Xiaozhihugan capsule,determine the VTQ values of three groups again,and compare the VTQ values of each group before and after treatment. Results：There was no significant difference in the detection rate of fatty liver patients with different degrees of nonalcoholic fatty liver disease by using two methods of ARFI and CT. There were statistically significant differences of VTQ among the three groups before and after the use of Xiaozhihugan capsule. Conclusion：ARFI can be used to quantify the diagnosis of nonalcoholic fatty liver disease and evaluate the effect of Xiaozhihugan capsule.
