胰腺实性假乳头状瘤的超声及CT表现

摘　　要：目的分析胰腺实性假乳头状瘤(SPTP)的超声及CT表现。方法回顾性分析经手术病理学检查证实的11例胰腺实性假乳头状瘤的影像资料,11例均接受超声检查,8例同时行CT扫描。结果超声显示6例呈囊实性,含纤维包膜,与周围正常胰腺分解清楚; 5例呈实性,无包膜; 4例显示钙化;多普勒超声显示4例有少量至中量血流。同时行CT检查的患者,CT平扫显示5例为囊实性混杂密度肿块,3例为实性,2例显示钙化,动脉期实性部分呈现轻度强化,静脉期及延迟期实性部分、壁结节及包膜进一步强化,囊性部分无强化。结论胰腺实性假乳头状瘤的超声及CT表现具有一定的特征性,熟悉这些影像表现有助于准确诊断。Objective To analyze the sonographic and CT imaging features of solid pseudopapillary tumor of the pancreas （SPTP）. Methods hnages of 11 histopathologically proven solid pseudopapillary tumors （SPTs） in the pancreas were retrospectively reviewed. All 11 patients underwent ultrasound, and eight underwent CT. Results On ultrasound images, six lesions had mixed solid components and cystic areas and were separated from surrounding pancreatic tissue by a fibrous capsule. Five lesions were solid appearances and noneneapsulated. Varied patterns of calcification were seen in four tumors. Doppler ultrasound imaging showed small to moderate blood flow in four of eleven cases. As for eight of 11 lesions with CT, five were composed of a mixture of cystic and solid components, three were solid appearances, calcification was seen in two tumors. The solid portion demonstrated slight enhancement in the arterial phase and progressive enhancement in the portal venous phase and delayed phase on multiphasie CT scan. The cystic portions showed no enhancement. Conclusion Sonographie and CT manifestations of solid pseudopapillary tumors of the pancreas are specific, familiarity with their imaging appearances were helpful in making an accurate diagnosis.
