摘 要:目的:探讨攀枝花地区食物不耐受与皮肤过敏性疾病的相关性。方法:对攀枝花地区某医院皮肤科就诊确诊为皮肤过敏性疾病的815例患者进行检测,根据检测结果,将其分为慢性荨麻疹组(n=382)、慢性湿疹组(n=329)、异位性皮炎组(n=104),并选取50例同期入院体检的健康人作为正常对照组。采用酶联免疫吸附测定(ELISA)法检测三组皮肤病患者以及正常对照组受试者血清中7种食物(鸡蛋、虾、牛奶、大豆、牛肉、小麦、鳕鱼)特异性Ig G抗体;采用Pearson相关分析攀枝花地区食物不耐受情况与Ig G浓度和抗体阳性项目数量的相关性。结果:慢性荨麻疹组、慢性湿疹组、异位性皮炎组受试者的食物不耐受特异性Ig G阳性率均明显高于正常对照组,差异存在统计学意义(P <0. 05);慢性荨麻疹以及慢性湿疹患者,主要是以轻、中度不耐受,而异位性皮炎患者主要为重度不耐受,且Pearson相关分析结果显示,食物不耐受程度与Ig G浓度、抗体阳性项目数量之间正向相关(P=0. 014; P=0. 022); 7种食物中,四组受试者均以鸡蛋不耐受的阳性率最高;Pearson相关分析结果显示,食物不耐受情况与Ig G浓度和抗体阳性项目数量的呈正相关(P <0. 05)。结论:本地区食物不耐受情况与皮肤过敏性疾病密切相关,可根据食物的不耐受种类排序以及严重程度对本地区皮肤过敏性疾病患者的饮食进行指导。Objective: To investigate the correlation between food intolerance and skin allergic disease in Panzhihua. Methods: Detection of 815 patients with allergic skin disease in panzhihua,according to the test results,patients are divided into: chronic urticaria group( n = 382),chronic eczema group( n = 329),ectopic dermatitis group( n = 104),50 cases of healthy people were selected as normal control groups. Using the Enzyme linked immunosorption assay( ELISA) method to detect the specific IgG antibodies of seven types of foods( eggs,shrimp,milk,soy,beef,wheat and cod) in the serum,the association between the food intolerance and IgG concentrations,number of antibody positive items were analyzed by Pearson. Results: Chronic urticaria,chronic eczema,and atopic dermatitis group were significantly higher in food intolerance than normal control group( P〈0. 05); Chronic urticaria and chronic eczema are mainly due to light,moderate intolerance,whereas heterotopic dermatitis is mainly severe and intolerant,and the results of Pearson's analysis showed that food intolerance was positively correlated with IgG concentration and the number of antibody positive items( P =0. 014; P = 0. 022). Conclusion: Food intolerance in this region is closely related to skin allergies; it can be sorted according to the food's intolerance and severity to guide the diet of patients with skin allergic disease in the region
