开窗减压术治疗不同分型下颌骨牙源性角化囊性瘤的疗效分析

摘　　要：目的评价应用开窗减压术治疗下颌骨大型牙源性角化囊性瘤的临床效果。方法选取2013年6月至2017年6月本院收治的21例下颌骨大型牙源性角化囊性瘤患者均行开窗减压术治疗,对全部患者定期访随,进行临床和影像学检查,评估治疗效果。结果开窗术后减压时间为6~24个月,临床和影像学检查显示,囊腔周围有新骨形成,囊腔缩小明显,颌骨外形改建,牙齿倾斜移位部分纠正。单房型牙源性角化囊性瘤和多房型疗效相比差异无显著性(P>0.05)。二期刮除术后随访6~36个月,全部随访病例无复发。结论开窗减压术是治疗下颌骨大型牙源性角化囊性瘤安全、有效的方法,能很好地保存颌骨的外形及功能,减小手术创伤。Objective To evaluate the clinical effect of decompression in the treatment of large mandibular keratocystic odontogenic tumor. Method From June 2013 to June 2017, 21 patients with large mandibular keratocystic odontogenic tumor undergoing fenestration and decompression in our hospital were retrospectively analyzed. All the patients were followed up regularly. Clinical and X-ray examinations were performed to assess the effect of decompression. Result The duration of fenestration and decompression was 6 to 24 months. Clinical and radiographic examinations showed that new bone formed around the cyst, the cyst was significantly reduced, jaw shape was rebuilt, and the tilt of the tooth was partially corrected. All cases were followed up for 6-36 months after two stage curettage, and no recurrence was found in all cases. Conclusion Fenestration and decompression is a safe and effective method for the treatment of large mandibular keratocystic odontogenic tumor of the jaws. It can well preserve the shape and function of the jaws and reduce operative trauma.
