microRNA-200a在恶性肝脏肿瘤患者血液及肿瘤组织中的表达及其与预后的关系

摘　　要：目的探讨microRNA-200a在血液及肿瘤组织中的表达及其与恶性肝脏肿瘤患者预后的关系。方法选取79例行肝脏肿瘤切除术患者的病历资料,记录肝癌组织、癌旁组织、正常肝脏组织及血液样本中microRNA-200a相对表达量,并分析其与患者临床特征的关系。选择无肝胆原发疾病或影响肝功能的其他疾病且无肿瘤的83例患者为对照组,比较肝癌患者与对照组患者血清microRNA-200a的差异。根据肝癌组织microRNA-200a相对表达量构建预测患者结局的ROC曲线,通过约登指数确定的切点将患者分为高表达和低表达两组,分析恶性肝脏肿瘤患者生存情况。结果肝癌组患者血清microRNA-200a相对表达量明显低于对照组(t=9.599,P﹤0.01);肝癌组患者的肝癌组织、癌旁组织及正常肝脏组织的microRNA-200a相对表达量比较,差异有统计学意义(P﹤0.01)。高中分化及未发生转移的恶性肝脏肿瘤患者肝癌组织中microRNA-200a相对表达量均明显高于低分化及发生转移的患者,差异均有统计学意义(P﹤0.01);AFP水平≤8.1 ng/L、高中分化、未发生转移及TNM分期为Ⅰ~Ⅱ期的恶性肝脏肿瘤患者血清中microRNA-200a相对表达量均明显高于AFP水平﹥8.1 ng/L、低分化、发生转移及TNM分期为Ⅲ~Ⅳ期的患者,差异均有统计学意义(P﹤0.01)。死亡组患者肝癌组织microRNA-200a相对表达量明显低于生存组(t=3.878,P﹤0.01)。microRNA-200a高表达组患者生存情况明显优于低表达组(χ2=15.090,P﹤0.01)。结论 microRNA-200a与恶性肝脏肿瘤的生长增殖存在一定的关系,microRNA-200a表达水平越高,患者预后越好。Objective To explore the relationship between the expression of microRNA-200 a in blood and tumor tissue and the prognosis of patients with malignant liver tumors. Method The clinical information of 79 patients who underwent concurrent hepatectomy were collected. The relative expressions of microRNA-200 a in liver cancer tissue, adjacent normal tissue, normal liver tissue and blood sample were recorded, and their relationship with clinical features of patients were analyzed. Eighty-three patients without primary hepatobiliary disease or other diseases affecting liver function were selected as the control group, and the serum microRNA-200 a levels of control group were compared with those of the hepatocellular carcinoma patients. The ROC curve was constructed based on the relative expression levels of microRNA-200 a in hepatocellular carcinoma（HCC） tissues. The patients were divided into high expression group and low expression group according to the cutoff point determined by Youden index, and the survival of patients with malignant liver tumor were analyzed. Result The serum level of microRNA-200 a in HCC patients was significantly lower than that of the control group（t=9.599, P0.01）. The microRNA-200 a expression levels were significantly different among the HCC tissue, adjacent normal tissue and normal liver tissue（P0.01）. The relative expression of microRNA-200 a was significantly higher in patients with high grade differentiation and no metastasis than that in patients with low differentiation and metastasis（P0.01）. The relative expressions of microRNA-200 a in patients with malignant liver tumors of AFP≤8.1 ng/L, high grade differentiation, no metastasis and stage I-II of TNM was higher than those in patients with AFP8.1 ng/L, low differentiation, metastasis and TNM stage Ⅲ-Ⅳ, and the differences were statistically significant（P0.01）.The relative expression level of microRNA-200 a in the death group was significantly lower than that in the survival group（t=3.878, P0.01）.
