结直肠癌患者IL-6、TNF-α、CRP的水平变化特点及临床意义

摘　　要：目的观察结直肠癌患者的白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)的水平变化,并分析其临床意义。方法选取160例结直肠癌患者为结直肠癌组,选取160例健康体检者为健康对照组。对两组IL-6、TNF-α、CRP水平进行比较,观察IL-6、TNF-α、CRP水平与结直肠癌患者临床特征的关系,并对影响结直肠癌患者IL-6、TNF-α、CRP水平的因素进行分析。结果结直肠癌组患者的IL-6、TNF-α、CRP水平均明显高于健康对照组(P﹤0.01);IL-6、TNF-α、CRP水平均与结直肠癌患者的淋巴结转移情况、组织学分型、TNM分期、肿瘤直径有关(P﹤0.01),但均与结直肠癌患者的年龄、性别、肿瘤部位无关(P﹥0.05);多重逐步回归分析结果显示:淋巴结转移、TNM分期是影响结直肠癌患者IL-6、TNF-α、CRP水平的独立危险因素(P﹤0.01)。结论结直肠癌患者的IL-6、TNF-α、CRP水平较高,且与淋巴结转移和TNM分期密切相关。Objective To observe the level of interleukin-6（IL-6）, tumor necrosis factor-α（TNF-α） and C-reaction protein（CRP） in patients with colorectal cancer and to analyze the clinical significance. Method 160 cases of colorectal cancer were included in the study as colorectal cancer group, and 160 healthy subjects were recruited as healthy control group. The levels of IL-6, TNF-α and CRP were compared between the two groups, and the relationship between the levels of IL-6, TNF-α, CRP and the clinical features of colorectal cancer was observed, besides, the factors influencing the levels of IL-6, TNF-α and CRP were analyzed. Result The levels of IL-6, TNF-α and CRP in the patients with colorectal cancer group were all higher than those in healthy control group（P0.01）, and were associated with lymph node metastasis, histological type, TNM staging, and tumor size（P0.01）, but were independent of age, gender or tumor location（P0.05）; multiple stepwise regression analysis showed that lymph node metastasis and TNM staging were independent risk factors for IL-6, TNF-α and CRP levels in colorectal cancer patients（P0.01）. Conclusion The levels of IL-6, TNF-α,and CRP in patients with colorectal cancer are high, and are closely related to lymph node metastasis and TNM staging.
