小切口Phaco术、IOL植入并房角分离三联术治疗白内障并青光眼的临床效果

摘　　要：目的观察对白内障合并闭角型青光眼患者采用小切口超声乳化摘除(Phaco)术+人工晶状体(IOL)植入并房角分离术进行治疗的临床效果。方法将在我院接受手术治疗的120例白内障并青光眼患者按照随机数字表法分为对照组和观察组,各60例。其中对照组接受小切口Phaco术+IOL植入联合小梁切除术,观察组接受小切口Phaco术+IOL植入并房角分离术。治疗结束后,比较两组患者的眼内压、最佳矫正视力和中央前房深度的改善情况及并发症发生情况。结果术后1周、1、3、6个月时,两组患者的眼内压均显著降低,最佳矫正视力及中央前房深度均显著改善(P<0.05),且观察组患者的最佳矫正视力及中央前房深度均优于对照组(P<0.05)。两组并发症情况比较,差异无统计学意义(P>0.05)。结论小切口Phaco术、IOL植入并房角分离三联术治疗白内障并青光眼安全有效,能够有效降低眼压,获得更好的视力和更佳的中央前房深度。Objective To observe the clinical effect of small incision phacoemulsification （Phaco） combined withintraocular lens （IOL） implantation and goniosynechialysis for cataract and angle closure glaucoma. Methods A total of 120patients with cataract and glaucoma treated in our hospital were divided into control group and observation group accordingto random digital table method, with 60 cases in each group. The control group received small incision Phaco combinedwith IOL implantation and trabeculectomy. The observation group received small incision Phaco combined with IOLimplantation and goniosynechialysis. After the treatment, the improvement of intraocular pressure, best corrected visualacuity and central anterior chamber depth, complications were compared between the two groups. Results At 1 week, 1,3and 6 months after operation, the intraocular pressure of both groups decreased significantly, the best corrected visualacuity and central anterior chamber depth significantly improved （P〈0.05）; the best corrected visual acuity and centralanterior chamber depth in the observation group were better than those in the control group （P〈0.05）. There was nosignificant difference in complications between the two groups （P〉0.05）. Conclusion Small incision Phaco combined withIOL implantation and goniosynechialysis are safe and effective for the treatment of cataract and glaucoma. It can effectivelyreduce intraocular pressure and achieve better vision and central anterior chamber depth.
