摘  要:目的探讨叶酸受体介导的宫颈染色检测宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)的临床价值。方法对2015年3月~2017年5月宫颈癌筛查提示传统宫颈刮片≥巴氏Ⅱ级519例,采用叶酸受体介导肿瘤诊断(folate receptor-mediated tumor diagnosis,FRD)进行宫颈染色和阴道镜下活检。结果宫颈活检诊断176例无CIN,121例CIN1,104例CIN2,81例CIN3,37例宫颈癌。FRD阳性诊断CIN2及以上的敏感度为84.7%(188/222),特异性76.8%(228/297),阳性预测值73.2%(188/257),阴性预测值87.0%(228/262),准确率80.2%(416/519)。结论 FRD在快速筛查宫颈上皮内瘤变中有临床价值。Objective To evaluate the feasibility of folate receptor-mediated tumor diagnosis (FRD) staining in screening for cervical intraepithelial neoplasia (CIN). Methods A total of 519 cases with abnormal Papanieolaou test( ≥grade Ⅱ ) in cervical cytological screening were given FRD staining and biopsy under eolposeopy from March 2015 to May 2017. Results Cervical biopsy showed no CIN in 176 cases, CIN1 in 121 cases, CIN2 in 104 cases, CIN3 in 81 cases, and cervical cancer in 37 cases. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of the FRD staining for CIN2 and above were 84.7% (188/222), 76. 8% (228/297), 73. 2% (188/257), 87. 0% (228/262), and 80. 2% (416/519), respectively. Conclusion FRD staining is feasible in screening for CIN.
