经皮气管切开术在ICU重症脑卒中患者中的临床应用探讨

摘　　要：目的探讨经皮气管切开术在ICU重症脑卒中患者中的临床应用效果。方法以我院ICU需要进行气管切开手术的重症脑卒中患者52例作为研究对象,采用单双号法随机分为观察组26例,对照组26例。观察组采用经皮气管切开术,对照组采用传统气管切开术,对比2组患者手术各项指标以及并发症发生情况。结果观察组手术时长、切口长度、出血量分别为(12.3±3.4)min、(15.6±2.8)mm、(16.2±5.9)mL,对照组分别为(25.7±7.1)min、(53.6±11.3)mm、(45.0±12.0)mL,差异显著(P<0.05);观察组术后并发症发生率为3.8%,低于对照组的30.8%;观察组ICU住院时间为(9.5±1.2)d,短于对照组的(13.1±1.0)d,差异存在统计学意义(P<0.05)。结论 ICU重症脑卒中患者使用经皮气管切开术,可缩短手术时长,减少切口长度及出血量,且ICU平均住院时间短,安全性较高,值得推广。Objective To investigate the clinical application of percutaneous tracheotomy in patients withsevere stroke in ICU. Methods From January 2017 to March 2018,52 patients in our hospital who underwenttracheotomy were randomly divided into observation group （26 cases）and control group （26 cases） .The observationgroup was treated with percutaneous tracheotomy,and the control group was treated with conventional tracheotomy. Theindexes and complications of the two groups were compared. Results The length of operation,length of incision andamount of bleeding in the observation group were （12.3±3.4） min,（15.6±2.8）mm,（16.2±5.9）mL,and theoperation time, incision length and blood loss（25.7±7.1）in the control group.Min,（53.6±11.3）mm,（45.0±12.0）mL; Thepostoperative complication rate in the observation group was 3.8% lower than that in the control group, the observationgroup ICU hospitalization time was （9.5±1.2）d shorter than the control group（13.1±1.0） d,and the difference wasstatistically significant（P〈0.05） .Conclusion The percutaneous tracheotomy for ICU patients with severe stroke has higher clinical value,better duration of operation, length of incision and bleeding volume. The average length of hospitalstay in ICU is short and the safety is high. It is worthy of promotion.
