超声血管造影在屈光间质混浊情况下对增殖性糖尿病性视网膜病变的诊断价值

摘　　要：目的探讨超声血管造影在屈光间质混浊情况下对增殖性糖尿病性视网膜病变玻璃体积血、视网膜脱离和新生血管膜的诊断价值。方法对纳入研究的对象行超声血管造影检查,而后得出造影诊断,所有对象行玻璃体切割术,将超声血管造影结果与术中直视下诊断结果对比,评估超声血管造影诊断的准确性。结果术前超声血管造影诊断为PDR玻璃体积血、PDR视网膜脱离与PDR新生血管膜的患者分别为3眼、10眼、19眼,术中直视下明确诊断为PDR玻璃体积血、PDR视网膜脱离与PDR新生血管膜的患者分别为2眼、12眼、18眼,超声血管造影针对三种病变的诊断准确率分别为96.88%、87.5%、93.75%。结论超声血管造影在屈光间质浑浊情况下对PDR的诊断有较高的诊断准确率,可为临床提供依据,以便医师进行预后评估及在术前采取相应措施,改善预后,有重要临床价值。Objective To evaluate the diagnostic value of ultrasound angiography in PDR vitreous hemorrhage,retinal detachment and neovascularization in the case of refractive stromal opacity.Methods Subjects included in the study were examined by ultrasound angiography,which was then diagnosed.Vitrectomy was performed on all objects.The accuracy of ultrasound angiography was evaluated by comparing the results of ultrasound angiography with that of intraoperative direct diagnosis.Results Preoperative ultrasound angiography diagnosis of PDR vitreous hemorrhage,PDR retinal detachment and PDR neovascularization were 3 eyes,10 eyes,19 eyes,respectively.Under direct vision,2 eyes,12 eyes and 18 eyes were eventually diagnosed with PDR vitreous hemorrhage,PDR retinal detachment and PDR neovascular membrane respectively.The diagnostic accuracy of ultrasound angiography for three lesions was 96.88%,87.5% and 93.75% respectively.Conclusion Ultrasound angiography has a high diagnostic accuracy for the diagnosis of PDR under the condition of refractive stroma turbidity. It can provide a basis for clinical diagnosis,so that the doctors can evaluate the prognosis and take appropriate measures to improve the prognosis before the operation,which has important clinical value.
