摘  要:目的探讨高温和噪声联合作用对工人听力异常的影响。方法采用文献计量学法,通过中国生物医学文献数据库、中国知网、万方数据资源系统、维普数据库、Pub Med、Web of Science和Google等学术检索系统,检索、收集、筛选1990—2017年发表的关于高温和噪声接触对工人听力影响研究的文献。采用Stata 12. 0软件对入选文献进行Meta分析,采用亚组分析探讨研究异质性来源;采用Begg法和Egger’s法检验研究发表偏倚。结果共纳入12篇文献,覆盖我国9个省份,涉及联合作用组工人1 668例,单纯噪声组工人1 544例。随机效应模型显示,高温合并噪声接触工人出现听力异常的风险是单纯接触噪声者的2. 09倍(比值比为2. 09,95%可信区间为1. 43~3. 04)。亚组分析显示,不同地区、诊断标准和文献质量评分可能为不同研究异质性因素来源。Begg法和Egger’s法检验结果显示,本研究无明显发表偏倚(P> 0. 05)。敏感性分析显示,研究结果具有稳定性及可靠性。结论高温和噪声联合接触工人发生听力异常的风险高于单纯接触噪声者。Objective To explore the effect of high temperature combined with noise exposure on hearing abnormality in workers. Methods The bibliometrics method was used to search,collect and screen related published articles from 1990 to 2017 through the Chinese Biology Medicine Database,Chinese National Knowledge Infrastructure,Wangfang Data,China and Science and Technology Periodical Database,Pub Med,Web of Science and Google. Meta-analysis was performed on the selected literature using Stata 12. 0 software. Sub-group analysis was used to study the source of heterogeneity; Begg's method and Egger's method were used to test the publication bias. Results A total of 12 articles were included,covering 9 provinces in China,involving 1 668 cases in combined group and 1 544 cases in noise group.The random effect model showed that the risk of hearing abnormalities in workers exposed to high temperature combined with noise was 2. 09 times of that in the noise group( odds ratio = 2. 09,95% confidence interval: 1. 43-3. 04). Subgroup analysis showed that different areas,different diagnostic criteria,and the quality of literature may be the sources of different heterogeneous factors. The results of Begg method and Egger's method tests showed that there was no obvious publication bias in this study( P〈0. 05). Sensitivity analysis showed that the results of this study are stable and reliable.Conclusion The risk of hearing abnormalities in workers exposed to high temperature combined with noise is higher than that of workers exposed to noise only.
