摘 要:目的探讨甚高频B型超声仪下毛母质瘤的声像图模式。方法回顾性分析34例经手术及病理证实为毛母质瘤患者的甚高频超声声像图资料,总结该病的甚高频超声仪检测下毛母质瘤的声像图模式,并用152例新患者154处皮损来验证该声像图模式对于检测毛母质瘤的敏感性和特异性。结果甚高频超声仪检测下毛母质瘤声像图表现为分布于头面部或颈部的真皮深层及皮下低回声病灶,其长轴与身体长轴一致,且内部伴有钙化、周边低回声晕及后方声影,且该钙化灶多在0. 08~0. 29mm之间,瘤体距离表皮大致为0. 45~5mm,瘤体测量(长×宽)大致为8~12mm×3~8mm。其对于检测毛母质瘤的敏感性和特异性分别为85. 71%(42/49)和88. 35%(91/103)。结论甚高频超声仪检测毛母质瘤具有较高的临床应用价值。Objective To explore the ultrasonographic model of pilomatricoma under the veryhigh-frequency(VHF) B type ultrasonoscope. Methods The VHF ultrasonographic data of 34 patients who were diagnosed as pilomatricoma via surgeries and pathologies were analyzed. The ultrasonographic model of pilomatricoma under the VHF ultrasonoscope was summarized,the detecting of sensitivity and specificity of pilomatricoma was verified based on 152 new patients' 154 skin lesions. Results Pilomatricoma under the VHF distributed in the deep dermis and subcutaneously hypoechoic niduses on the head or neck,and their long axes were consistent with the body' s long axes.There were calcification,with the length of 0. 08 ~ 0. 29 mm,peripheral hypoechoic halo and posterior shadowing in inner niduses; and the distance between the tumors and epidermis was 0. 45 ~ 5 mm,the size of tumor was about 8 ~ 12 mm × 3 ~ 8 mm. The sensitivity and specificity of the ultrasonographic model for pilomatricoma were85. 71%(42/49) and 88. 35%(91/103),respectively. Conclusion The VHF ultrasonoscope has a high clinical value on detecting pilomatricoma.
