摘　　要：目的:探究血清克隆性免疫球蛋白检查在慢性淋巴细胞白血病患者中的预后价值。方法:回顾性分析56例初步诊断为慢性淋巴细胞白血病患者,采用全自动电泳仪以及扫描仪观察并记录患者血清克隆性免疫球蛋白(Ig)的表达情况,并探究血清克隆性Ig的表达情况与慢性淋巴细胞白血病预后因素的影响。结果:56例慢性淋巴细胞白血病患者中11例为克隆性Ig,克隆性Ig G者6例,Ig M者4例,Ig A者1例;血清克隆性Ig的表达情况与慢性淋巴细胞白血病的Binet分期、血清TK1的水平及FISH del(11q22. 3)存在一定的相关性。结论:大部分慢性淋巴细胞白血病患者血清中存在克隆性免疫球蛋白,通过血清克隆性免疫球蛋白的有无可作为慢性淋巴细胞白血病预后研究指标。Objective To explore the prognostic value of serum clonal immunoglobulin in patients with chronic lymphoblastic leukenfia. Method The expression of clonal immunoglobulin （Ig） in serum of 56 patients with newly diagnosed chronic lympho- blastie leukenfia was analyzed retrospectively. The expression of elonal Ig in serum and chronic lymphocyte were observed and re- corded by automatic eleetrophoresis and scanning instrument. Prognostic factors of cell leukenfia. Results 11 of 56 patients with chronic lymphoblastie leukemia were elonal Ig,6 eases were elonal IgG,4 eases were IgM,and 1 eases was IgA. The expression of clonal Ig in serum was correlated with Binet stage, serum TK1 level and FISH del （11q22. 3 ）. Conclusion Clonal immunoglobu- lin exists in the serum of most patients with chronic lymphoblastie leukemia, which is expressed by elonal immunoglobulin in ser- um. The presence of protein can be used as a prognostic indicator for chronic lymphocytic leukemia.
