MDR1基因C3435T多态性对精神分裂症帕利哌酮临床疗效的影响

摘　　要：探讨精神分裂症患者多药耐药MDR1基因C3435T多态性与帕利哌酮临床疗效的关系.对124例精神分裂症患者使用帕利哌酮缓释剂治疗8周,测定PANSS量表,并根据PANSS量表及分量表的减分率将患者分别分为总疗效、阳性症状疗效及阴性症状疗效的有效组和无效组;采用LDR-PCR技术检测MDR1基因C3435T位点的基因型.精神分裂患者中CC、CT、TT所占比例分别为24.2%、60.5%、15.3%,其中CT所占比例明显高于CC和TT基因型(P<0.05),C等位基因与T等位基因所占比例分别为54.4%和45.6%(P<0.05).帕利哌酮治疗总疗效、阳性症状疗效及阴性症状疗效的有效组与无效组相比,C3435T基因型CC、CT与TT及等位基因C、T的分布均无显著性差异(P>0.05).MDR1基因C3435T多态性可能与帕利哌酮缓释剂治疗精神分裂症的临床疗效无关.To investigate the association of MDR1 gene C3435T polymorphisms with clinical response of paliperidone extended-release tablets treatment in schizophrenia. 124 cases of schizophrenic patients were chosen to treat with paliperidone extended-release tablets for 8 weeks. The efficacy of paliperidone medication was evaluated by PANSS scale. The patients were divided into two groups according to the reduced rate of PANSS. Allelic typing of MDR1 was detected by LDR-PCR technique. The distribution of CC,CT and TT genotypes were 24.2%,60.5% and 15.3% in schizophrenia patients respectively. The proportion of CT genotype was significantly higher than that of CC and TT （P〈0.05）.The proportion of C and T allele were 54.4% and 45.6%, respectively （P〈0.05）.The distributions of genotype CC,CT,TT and allele C,T had no significant differences between the response group and the non-response group to paliperidone extended-release tablets in clinical efficacy of the total score of PANSS and positive symptoms and negative symptoms （P〉0.05）. The polymorphisms of MDR1 gene C3435T may not relate with clinieal response to paliperidone on schizophrenia patients.
