摘 要:目的探讨吞咽训练对老年鼻咽癌放疗病人吞咽功能和生活质量的影响。方法 56例接受根治性放疗的老年鼻咽癌病人随机分为吞咽训练组(28例)和对照组(28例),对照组给予常规的健康教育,吞咽训练组在对照组基础上给予吞咽功能训练,比较2组放疗前、放疗后1个月、放疗后6个月的吞咽功能和生活质量。结果放疗后1个月,对照组的吞咽功能评分和生活质量评分均低于放疗前;放疗后1个月、6个月,吞咽训练组的吞咽功能评分均高于对照组,生活质量评分均高于其放疗前和对照组。结论吞咽训练可以改善老年鼻咽癌放疗病人的吞咽功能,提高病人的生活质量。Objective To explore the effect of swallowing training on swallowing function and quality of life in the elderly patients with nasopharygeal carcinoma receiving radiotherapy. Methods Fifty-six elderly patients with nasopharygeal carcinoma receiving radical radiotherapy were randomly divided into the swallowing training group and the control group( CG),with 28 patients in each group.The control group was given conventional health education,and the swallowing training group was given swallowing function training on the basis of control group. The swallowing function assessed by M. D.Anderson dysphagia inventory( MDADI) and the quality of life assessed with quality of life questionnairecore 30( QLQ-c30) were compared between two groups before radiotherapy,1 month and 6 months after radiotherapy. Results The scores of MDADI and QLQ-c30 in the control group 1 month after radiotherapy were significantly lower than those before radiotherapy. MDADI score in the swallowing training group 1 and 6 months after radiotherapy was significantly higher than that in the control group. 1 and 6 months after radiotherapy,the score of QLQ-c30 in the swallowing training group was significantly higher than that before radiotherapy and that in the control group. Conclusions Swallowing training can effectively improve the swallowing function and the quality of life in the elderly patients with nasopharygeal carcinoma receiving radiotherapy.
