从脑卒中患者管理现状看个案管理师介入空间

摘　　要：了解脑卒中患者管理现状,分析医护技管理现存不足的原因,探索解决问题的有效办法。通过自行设计脑卒中医护技工作开展现状调查问卷,分别对医生护士康复技师进行问卷调查。结果显示医护人员对卒中患者危险因素、并发症、早期康复、出院计划及医患沟通方面的管理普遍较差。未配备专职人员被认为是影响危险因素管理(74.02%)、康复实施(72.83%)的重要因素。59.84%的医务人员希望配备专职人员协调监督出院准备计划的实施。笔者认为个案管理师可以填补脑卒中管理不足的现状,发挥需求评估、计划协调、服务管理监控、资源开发与照护咨询教育的作用。To understand the current situation of the management of stroke patients, analyze the reasons for the shortage of health care technology management, and explore effective ways to solve the problem. A self-designed questionnaire was conducted to investigate the status quo of TCM nursing skills in cerebral apoplexy. The management of risk factors, complications, early rehabilitation, discharge plan and doctor-patient communication of medical staff was generally poor. The lack of full-time staff is considered to be an important factor influencing risk factor management （74.02%） and rehabilitation implementation （72.83%）. 59.84% of the medical staff would like to provide full-time personnel to coordinate and supervise the implementation of the discharge plan.The case manager can fill the current situation of insufficient nursing management of cerebral pawns, and give full play to the role of demand assessment, plan coordination, service management monitoring, resource development and care consultation education.
