甲状腺癌患者术后两种镇痛方案的镇痛效果观察

摘　　要：目的:观察两种镇痛方案对甲状腺癌患者术后镇痛质量的影响。方法:选择全身麻醉甲状腺癌手术患者60例,随机均分为两组。观察组镇痛方案为颈丛神经阻滞联合静脉自控镇痛(PCIA),对照组镇痛方案为PCIA。评价记录术后1、6、12、24、48 h视觉模拟镇痛评分(VAS),48 h镇痛泵按压次数,术后头晕、恶心、呕吐发生率及术后恢复质量(QoR-15)评分。结果:观察组VAS评分、48 h镇痛泵按压次数均低于对照组,患者术后QoR-15评分高于对照组,差异有统计学意义;术后头晕、恶心、呕吐发生率两组比较差异无统计学意义。结论:颈丛神经阻滞联合PCIA的镇痛方案用于甲状腺癌根治术患者镇痛效果满意,提高了患者术后恢复质量,是有效的疼痛管理方案。Objective： To observe effect of two analgesia programs on thyroid caricinoma patients＇ analgesia quality in postoperative period. Methods： 60 patients with general anesthesia for thyroid carcinoma surgery were randomly divided into two groups. Analgesia program in observation group was cervical plexus block combined with patient-controlled intravenous analgesia（ PCIA）,and analgesia program in control group was PCIA. Evaluate and record visual analogue scale,VAS at 1 h,6 h,12 h,24 h,48 h after surgery,patient-controlled analgesia pressing times at 48 h,dizziness,nausea,vomiting incidence rate and postoperative recovery quality（ QoR-15） scale. Results： VAS and patient-controlled analgesia pressing times at 48 h in observation group was lower than control group,and postoperative recovery quality scale（ QoR-15） was higher than control group,and difference had statistical significance. There was no statistical significance for postoperative dizziness,nausea and vomiting incidence rate in both groups. Conclusion：Analgesia program of cervical plexus block combined with PCIA has satisfactory effect for patients who underwent thyroid caricinoma surgery,which improved patients＇ recovery quality in postoperative period,and it is the effective pain management program.
