摘 要:目的:系统评价环状RNA(circular RNA,circRNA)在中国人群消化道肿瘤中的诊断价值。方法:在Pub Med、Cochrane Library、Medline、Embase、万方、知网、维普等数据库检索,筛选2017年6月17日之前发表的可纳入的文献。利用诊断准确度的质量评价工具(QUADAS-2)对文献质量进行评价。采用STATA 14. 0软件对灵敏度和特异度进行合并,以及绘制、分析综合受试者工作曲线(SROC)及曲线下面积(AUC)。结果:按照纳入、排除标准,共纳入10篇文献。合并后的灵敏度和特异度为0. 68(95%CI:0. 57~0. 78),0. 76(95%CI:0. 68~0. 83),曲线下面积为0. 79(95%CI:0. 75~0. 82)。亚组分析示肿瘤类型和样本量是异质性的主要来源。Deeks'漏斗图示本研究存在发表偏倚(P=0. 01)。结论:circRNA具有成为诊断消化道肿瘤的新型生物标志物的潜能。Objective To evaluate the potential diagnostic value of circRNAs for gastrointestinal cancer in China. Methods: The relevant literature was identified in databases such as PubMed and Embase (up to June 17,2017),and the information from eligible studies was extracted.The quality of the studies included in this Meta-analysis was assessed by using the quality assessment of diagnostic accuracy studies (QUADAS-2) tool.The pooled sensitivity and specificity,summary receiver characteristic (SROC) curves and the area under curve (AUC) were performed.STATA 14.0 was used to perform this Meta-analysis. Results: According to the inclusion and exclusion criteria,10 studies were included in total.The overall sensitivity and specificity was 0.68 (95%CI:0.57~0.78) and 0.76 (95%CI:0.68~0.83),with an area under the SROC curve (AUC) of 0.79 (95%CI:0.75~0.82).Meta-regression analysis showed that cancer types was the main sources of heterogeneity.Deeks' funnel plot asymmetry test ( P =0.01) suggested that there is potential publication bias in the present study. Conclusion: Our Meta-analysis indicated that circRNAs has a great potential to be a novel biomarker in the diagnostic of digestive cancer.However,more well-designed studies with larger sample size on the diagnostic value of circRNAs for gastrointestinal and other cancers are needed in the future.
